17. AGREEMENT WITH KEOGH HEALTH CONNECTION FOR HEALTH CARE EDUCATION AND SERVICES Approve retroactively, an agreement between Maricopa County and Keogh Health Connection (Health Care Education and Services) to allow the provider to work with the Sheriff's Office and utilize jail facilities to educate and help inmates better understand health care options and health plan enrollment. The term of this agreement is November 1, 2014 through November 1, 2015. Keogh will provide all staffing and equipment needed to conduct these duties at no expense to the Sheriff's Office. Either party may terminate this agreement without cause with 7 days written notice. (C-50-15-052-M-00)

AMENDMENT TO AGREEMENT WITH KEOGH HEALTH CONNECTION FOR INMATE HEALTHCARE EDUCATION AND SERVICES Approve Amendment No. 1 to the Agreement between Maricopa County by and through the Maricopa County Sheriff’s Office and Keogh Health Connection. The purpose of this Amendment is to renew the Agreement for one (1) year from November 1, 2015 to November 1, 2016. All other terms and conditions of the Agreement remain in full force and effect. Approval of this item allows the provider to work with the Sheriff’s Office to educate and help inmates better understand health care options and mandated health care plan enrollment requirements. AMENDMENT TO AGREEMENT WITH KEOGH HEALTH CONNECTION FOR INMATE HEALTHCARE EDUCATION AND SERVICES Approve Amendment No. 2 to an Agreement between Keogh Health Connection (C-50-15-052-M-00) and Maricopa County through the Sheriff's Office. The purpose of this amendment is to renew the Agreement for one (1) year. This amendment is non-financial and is effective from November 1, 2016 until November 1, 2017. All other terms and conditions of the Agreement remain in full force and effect. Approval of this item allows the provider to work with the Sheriff's Office to educate and help inmates better understand health care options and mandated health care plan enrollment requirements.
