IGA WITH MARICOPA COUNTY HUMAN SERVICES DEPARTMENT FOR COMMUNITY ACTION PROGRAM CRISIS CASE MANAGEMENT ASSISTANCE SERVICES Approve an Intergovernmental Agreement (“Agreement”) between the Maricopa County Library District (“Library District”) and Maricopa County Human Services Department (“County”), Community Services Division (“Department”) pertaining to Community Action Program crisis case management assistance services. The County and the Library District collectively are referred to as the “Parties” and individually as the “Party”. The purpose of this agreement is to address the following: 1. Integrate County’s Community Action Program services into the Library District’s social and community services resource network. The Parties’ goal is to support the accessibi lity of crisis case management services to eligible residents within local communities. 2. The integrated service delivery model will be located at the following Library District facilities: a. Southeast Regional Library (775 N Greenfield Road, Gilbert, A Z 85234) b. Aguila Library (51300 US-60, Aguila, AZ 85320) c. Perry Library (1965 E Queen Creek Road, Gilbert, AZ 85297) d. Queen Creek Library (21802 S Ellsworth Road, Queen Creek, AZ 85142) 3. The Agreement may be extended as many times as is desirable, but each extension may not exceed the duration of the previous Agreement. This agreement does not have a financial impact.
