H-4. RESOLUTION 15-023 SERVICES AND SUPPORTS AGREEMENT BETWEEN MERCY MARICOPA INTEGRATED CARE AND THE HOUSING AUTHORITY OF MARICOPA COUNTY FOR THE BRIDGE TO PERMANENCY HOUSING PROGRAM Adopt Resolution 15-023 and approve the Housing Authority of Maricopa County to enter into the Services and Supports Program Agreement with Mercy Maricopa Integrated Care to become a Bridge Subsidy Program Participating Provider. This Agreement shall be effective November 1, 2015, and shall be effective for an initial term of one (1) year and will automatically renew for additional one (1) year terms unless and until terminated in accordance with Section 6.0 of the Agreement. HAMC will receive a maximum reimbursement of $561,542 for all costs and expenses for the time frame of November 1, 2015 to June 30, 2016. (C-66-16-008-M-00)
