EXTENSION TO CONTRACTS UNDER RFP 15039 FOR RYAN WHITE PART A SERVICES-EARLY INTERVENTION Authorize and approve an extension to Contracts under RFP 15039 for Ryan White Part A program services between Maricopa County through its Department of Public Health and: 1. Terros , Inc. 2. Southwest Center for HIV/AIDS 3. Area Agency on Aging The Department requires additional time to prepare for the upcoming RFP process due to updated requirements from Health Resources and Services Administration’s (HRSA) and HIV/AIDS Bureau (HAB) regarding Ryan White Part A funding. The contract term is extended for an additional three (3) months from March 1, 2021 through May 31, 2021. This is a 3-month extension beyond the original maximum 6-year term, making the total term 6 years, 3 months from March 1, 2015 thru May 31, 2021. The Base Contract and first extension were approved under Procurement Services. The Ryan White Part A Services program use these contracts to provide for early intervention support services for eligible individuals with HIV/AIDS. (C-73-16-007-7-01)

EXTENSION TO CONTRACT UNDER RFP 15039 FOR RYAN WHITE PART A SERVICES-EARLY INTERVENTION Approve an amendment to extend the RFP between Southwest Center for HIV/AIDS and Maricopa County Department of Public Health for the purpose of identifying HIV positive individuals at key points of entry, HIV counseling and to ensure that newly diagnosed individuals are linked to HIV Care and treatment. This extension will allow for the time needed to successful transition all newly HIV diagnosed individuals from the Southwest Center for HIV/AIDS Early Intervention Services program to other Ryan White Part A contracted services. The contract term is extended for an additional three (3) months from May 31, 2021 through August 30, 2021. This is an additional extension beyond the original maximum 6-year term, making the total term 6 years, 6 months from March 1, 2015 thru August 30, 2021. The final invoice is due to MCDPH on or before September 15, 2021. The Base Contract was approved under Procurement Services. (C-73-16-007-7-02)
