28. 12165-SS, SCOTTSDALE NURSING AND REHABILITATION CENTER This Contract provides long term care services including room and board for one patient. Human Services utilizes this contract for long term adult care, level two for a patient that the County has taken responsibility for because family could not be located. This contract contains options for contract renewals and is being retroactively renewed for a term of five years from January 1, 2023, thru December 31, 2027. The remaining MA balance in Advantage 3x of $177,505.38 is not sufficient to cover anticipated spend thru the end of this new term, therefore $390,000.00 needs to be added to the Contract award value.
