SITE SPECIFIC BUILDING ENTRY AGREEMENT WITH QWEST CORPORATION D/B/A CENTURYLINK QC Approve and authorize the Chairman to execute license P-50332, a Site Specific Building Entry Agreement between Qwest Corporation d/b/a CenturyLink QC, a Colorado corporation (“Licensee”) and Maricopa County, a political subdivision of the state of Arizona (“County”) for access to the County building located at 16821 N. Dysart Road, Surprise, AZ 85374 for installation of telecommunication services. The County desires to receive On-Net EPL Metro Ethernet Access Connection 100 baseT hand-off 70Mbs (“Service”) to the Building pursuant to that certain procurement contract known as Maricopa County Serial 05159-RFP Carrier Services, as amended by that certain CUSTOMER ENTRANCE BUILD ADDENDUM FOR CENTURYLINK SERVICE Pramata #1060964 which is being executed by the County Office of Procurement Services. This entry agreement will provide a license for Centurylink to access to the Property and Building to construct, maintain, repair, operate and remove, at Licensee’s sole option and expense, certain wires, cables, panels, conduit, lock-boxes, and other appurtenant fixtures and equipment in order to provide the service. The term of the Agreement is five (5) years and will automatically renew for two (2) consecutive five (5) year terms unless terminated by Licensee or County via written notice to the other party of its intent not to renew at least three (3) months prior to the end of any term. There is no cost to the County for this Agreement. (C-78-19-009-3-00)
