37. NEW HOPE AGREEMENT WITH PANALEA WELLNESS VILLAGE, INC. Approve an Agreement between Panalea Wellness Village, Inc., a 501 (C) 3 corporation, P O Box 1713, Higley, AZ 85236, and Maricopa County to allow Panalea Wellness Village, Inc. under the New Hope Program to rescue animals that have been deemed eligible for the New Hope program. Maricopa County will provide a rabies vaccination, dog license tag and new owner transfer fee within the first year of rescue for each dog three months of age or older at no cost to the Contractor. The cost for these services is $41 for each animal rescued. Animal Care & Control estimates 24 new hope rescues over the term of the agreement, for a total of $984. The term of this Agreement is from August 17, 2011 through June 30, 2014. (C-79-12-017-3-00)

26. AMENDMENT TO THE AGREEMENT WITH PANCEA ANIMAL WELLNESS SANCTUARY, INC Approve Amendment No. One to an Agreement between Panacea Animal Wellness Sanctuary, Inc. (C-79-12-017-3-00) and Maricopa County through the Animal Care & Control. The purpose of this amendment is to change CONTRACTOR's name. This amendment is non-financial and is effective from December 12, 2012 until June 30, 2014. All other terms and conditions of the Agreement remain in full force and effect. Amendment One changes the name of the Agreement CONTRACTOR, Panacea Wellness Village, Inc. and replaces it with panacea Animal Wellness Sanctuary, Inc. C-79-12-017-3-00 was approved August 17, 2011 by the Board of Supervisors. (C-79-12-017-3-01)
