KENNEL PERMIT APPLICATION FOR LUE JEANE BROOKS D/B/A SPRINGBROOK KENNELS Approve kennel permit K19-000004 pursuant to A.R.S. §11-1009 for Lue Jeane Brooks d/b/a Springbrook Kennels, 36614 N. 23rd Street, Phoenix, AZ 85086 in District 3 for a one-year period beginning the first day of the month following Board approval. The cost of a kennel permit is $350, effective June 1, 2018. This application was initiated prior to June 1, 2018, but not completed by that date through no fault of the parties. Therefore, the total amount received for this kennel permit application is $328, the cost of a permit prior to the June 1, 2018 fee increase. (C-79-19-031-3-00)
