IGA WITH MARICOPA COUNTY SPECIAL HEALTH CARE DISTRICT DBA VALLEYWISE HEALTH Approve and execute Intergovernmental Agreement between Maricopa County (“County) and Maricopa County Special Health Care District DBA Valleywise Health (“District”) G-30227, for the use of 1,200 square feet of Real Property located at 33 West Tamarisk Ave, Phoenix, Arizona 85041; and authorize the Assistant County Manager and/or the County Real Estate Director to administer the Agreement. The lease period begins on the date of execution by the parties and ends on December 31, 2026, (approximately five (5) years) and will automatically renew for two (2) subsequent five (5) year terms unless either party gives the other ninety (90) days written notice prior to the renewal. AMENDMENT TO IGA BETWEEN MARICOPA COUNTY AND MARICOPA COUNTY SPECIAL HEALTH CARE DISTRICT DBA VALLEYWISE HEALTH Approve and execute First Amendment to the Intergovernmental Agreement between Maricopa County (“County) and Maricopa County Special Health Care District DBA Valleywise Health (“District”) to grant County the right to sublease its rights to use the Monitor Site and property under the Agreement to any other governmental entity for the purpose of environmental monitoring. The Air Quality Monitoring site that is the subject of this First Amendment is being used by the County’s Air Quality Department and the purpose of this Amendment is to sublease a portion of the monitoring site to other governmental entities. There will be no fees charged for the use of this property. This site is located in Supervisory District 5 (C-85-22-023-X-01)

SUB-INTERGOVERNMENTAL AGREEMENT TO IGA FOR THE USE OF REAL PROPERTY AND IMPROVEMENTS BY AND BETWEEN MARICOPA COUNTY AND ARIZONA DEPARTMENT OF ENVIRONMENTAL QUALITY Approve and execute a Sub-Intergovernmental Agreement between Maricopa County (“County) and Arizona Department of Environmental Quality (“ADEQ”) for the use of 5 square feet within a 1,200 square foot Air Quality Monitoring Site located at 33 West Tamarisk Ave, Phoenix that the County is utilizing under an Intergovernmental Agreement (“Agreement”) with the Maricopa County Special Health Care District (dba Valleywise ) C-85-22-023-X-00. The term of the sub-agreement is coterminous with the Agreement. The sub-agreement term commences on the date of execution by the parties and expires on December 31, 2026. It will automatically renew for two (2) subsequent five (5) year terms unless either party gives the other party ninety (90) days written notice prior to the renewal.
