34. AMENDMENT TO LEASE AGREEMENT FOR IMMUNIZATION CLINIC AT BRITTANY SQUARE SHOPPING CENTER Approve Amendment No. 1 to Lease No. L-7343 with Binderup Investments, Inc., as successor in interest to Brittany Square Shopping Center , LLLP, limited liability limited partnership, Lessor, for premises currently used as an immunization clinic. The premises located at 6666 W. Peoria Ave , Glendale AZ , contains 2,368 square feet of leased space. The current lease is extended five years to commence on January 1, 2009 and terminating on December 31, 2013. The annual base rental cost are: Year 1) $34,335.96, Year 2) $35,520.00, Year 3) $36,704.00, Year 4) $38,480.00, and Year 5) $40,256.00, plus 2.4 % rental tax. Additional estimated rents for Common Area Charges (CAM) are $10,800.00 per year. Tenant is also responsible for the monthly utility charges and maintenance cost. The Lease Agreement contains provisions for lease termination with 180-day written notice. The Facility Review Committee has endorsed this lease amendment. All other terms and conditions of the Lease remain the same and in full force. (C-86-04-022-4-01)

ADMINISTRATIVE CORRECTION TO AGENDA FOR LEASE AGREEMENT WITH BRITTANY SQUARE SHOPPING CENTER Approve administrative correction to agenda item C-86-04-022-4-01, dated December 17, 2008, which approved Lease No. L-7343 with Brittany Square Shopping Center , LLLP, a limited liability limited partnership, for 2,368 square feet of clinic space at 6666 W. Peoria Ave. , Glendale , AZ. This correction specifies the complete title of the new landlord to said shopping center as Binderup Investments, Inc., DBA Arizona Brittany Square , LLC, as Lessor. All other terms and conditions of the lease remain the same, with the correction of ownership the only change. As such, this is an informational request. (C-86-04-022-4-02)

63. LEASE AGREEMENT L-7343 AMENDMENT WITH BINDERUP INVESTMENTS, INC. DBA ARIZONA BRITTANY SQUARE, LLC, INC FOR IMMUNIZATION SPACE Approve, and authorize the Chairman to execute, Amendment No. 2 to Lease Agreement L-7343 with Binderup Investments, Inc. DBA Arizona Brittany Square, LLC, for immunization clinic space at 6666 West Peoria Avenue, Glendale, AZ. Amendment No. 2 will extend the term of the agreement from January 1, 2014 through December 31, 2016. The lease rate will be $16.00 per square foot for year one with $0.50 per square foot yearly increases thereafter. Either party may terminate the agreement with a minimum of one hundred eighty-day (180) written notice. (C-86-04-022-4-03)

EARLY TERMINATION OF LEASE WITH BINDERUP INVESTMENTS, INC., DBA ARIZONA BRITTANY SQUARE LLC FOR IMMUNIZATION CLINIC Approve the early termination of Lease L-7343 and direct Real Estate Services staff to give written notice to Binderup Investments, Inc., dba Arizona Brittany Square, LLC, Lessor of the subject termination. Lease L-7343 is for approximately 2,368 s.f. of leased space for an Immunization Clinic at 6666 West Peoria Avenue in Glendale, AZ. For efficiency purposes, Public Health is relocating this Immunization Clinic into a combined lease premises with the Office of Vital Records. Section 10 of Lease Agreement L-7343 allows for early termination with 180-days written notice. (C-86-04-022-4-04)
