28. AMENDMENT TO LEASE WITH DAVISON FAMILY TRUST FOR WIC CLINIC SITE Approve and execute Amendment No. 2 to Lease No. L-7295 with Davison Family Trust, Lessor, for 2,560 square feet of office space located at 4419 E. Main, Mesa, Arizona. The term of the lease is extended five years, commencing July 7, 2010 and terminates July 6, 2015. The annual rental rate for 10-11 is at $15.00 s/f or $38,400.00 annually, for 11-12 at $15.45 s/f or $39,552.00 annually, for 12-13 at $15.91 s/f or $40,728.00 annually, for 13-14 at $16.39 s/f or $41,958.00 annually, for 14-15 at $16.88 s/f or $43,212.00 annually, plus rental tax. The County Facility Review Committee endorsed this Lease Amendment at their January meeting. (C-86-05-916-4-01)
