29. AMENDMENT TO AGREEMENT WITH SOUTHWEST CATHOLIC HEALTHCARE NETWORK D.B.A. MERCY CARE PLAN Approve Amendment No. 1 to Participating Health Provider Agreement between Southwest Catholic Healthcare Network d.b.a. Mercy Care Plan, an Arizona Health Care Cost Containment System (AHCCCS) health plan, and Maricopa County d.b.a. Healthcare for the Homeless through the Department of Public Health’s Health Care for the Homeless (HCH) Clinic. This amendment replaces Attachment B of the original contract and is effective June 1, 2008. All other terms and conditions of the original agreement remain in full force and effect.
