27. AMENDMENT TO IGA WITH MARICOPA COUNTY SPECIAL HEALTH CARE DISTRICT D.B.A. MARICOPA INTEGRATED HEALTH SYSTEMS D.B.A. MARICOPA HEALTH PLAN FOR HEALTH CARE FOR THE HOMELESS CLINIC Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) with Maricopa County Special Health Care District d.b.a. Maricopa Integrated Health Systems d.b.a. Maricopa Health Plan (MHP) and Maricopa County through its Department of Public Health’s Health Care for the Homeless Clinic (HCH). This IGA is effective October 1, 2008 and amends Exhibit 5: AHCCCS Subcontract Provisions. All other terms and conditions of the original Contract shall remain in full force and effect. This agreement is non-financial and will not affect the County general fund. (C-86-06-052-2-02)

AMENDMENT TO MOU WITH MARICOPA INTEGRATED HEALTH SYSTEMS DBA MARICOPA HEALTH PLAN FOR HEALTHCARE FOR THE HOMELESS CLINICAL SERVICES Approve Amendment No. 3 to the Memorandum of Understanding (MOU) with Maricopa Integrated Health Systems dba Maricopa Health Plan and Maricopa County by and through its Department of Public Health dba Maricopa County Department of Health Services dba Public Health Clinical Services. This Amendment serves to ensure the proper name is documented in our Agreement. The terms of this MOU is effective October 1, 2008 until terminated by either party. All other terms and conditions of the original Agreement shall remain in full force and effect. This Amendment is a non-financial agreement and will not affect the County General fund. AMENDMENT TO MOU WITH UNIVERSITY PHYSICIANS HEALTH CARE DBA THE UNIVERSITY OF ARIZONA HEALTH PLANS FOR HEALTHCARE FOR THE HOMELESS CLINICAL SERVICES Approve Amendment No. 4 to the Memorandum of Understanding (MOU) with University Physicians Health Care dba The University of Arizona Health Plans and Maricopa County by and through its Department of Public Health dba Maricopa County Department of Health Services dba Public Health Clinical Services. This Amendment serves to ensure the proper name is documented in our Agreement. The terms of this MOU is effective July 1, 2014 until terminated by either party. All other terms and conditions of the original Agreement shall remain in full force and effect. This Amendment is a non-financial agreement and will not affect the County General fund. 62. AMENDMENT TO AGREEMENT WITH MARICOPA INTEGRATED HEALTH SYSTEMS DBA MARICOPA HEALTH PLANS Approve a retroactive Amendment to Agreement between Maricopa Integrated Health Systems dba Maricopa Health Plans (MHP) and Maricopa County by and through its Department of Public Health, Maricopa County dba Maricopa County Department of Health Services, dba Public Health Services, Healthcare for the Homeless (HCH) for the Arizona Health Care Cost Containment System (AHCCCS) has modified the reimbursement methodology for AHCCCS registered Federally Qualified Health Centers/Rural Health Center’s (FQHC/RHC’s). This agreement shall become effective April 1st, 2015 and or the date set forth by AHCCCS. All other terms and conditions of the Agreement shall remain in effect. The retroactive is due to the process time for corrections with the provider. The Agreement allows HCH to bill Maricopa Integrated Health Systems dba Maricopa Health Plans (PHP), for reimbursable client services provided at the HCH Clinic. This allows HCH, a federally grant funded program, to generate additional revenue for the program. The overall fee fund budget will be adjusted as necessary to accommodate increase in revenue through a future reconciliation. (C-86-06-052-2-05)
