AMENDMENT TO SUBCONTRACT WITH HEALTH CHOICE ARIZONA Approve the Amendment to the Health Choice Arizona (HCA) subcontract to change the name from Maricopa County, D.B.A. Maricopa County Department of Health Services, D.B.A. Health Care for the Homeless to Maricopa County, D.B.A. Maricopa County Department of Health Services, D.B.A. Public Health Clinical. 76. AMENDMENT TO SUBCONTRACTOR AGREEMENT WITH HEALTH CHOICE ARIZONA Approve retroactive Amendment No. 2 to the Subcontract with Health Choice Arizona (HCA) between Maricopa County Department of Public Health (MCDPH), d.b.a. Maricopa County Department of Health Services d.b.a. Public Health Clinical. This Amendment will delete and replace, SUBCONTRACTOR AGREEMENT, Attachment B-1, SECTION 3 – FEE-FOR-SERVICE REIMBURSEMENT: For authorized Primary Care, and Dental Care Covered Services provided to Health Choice Arizona Members, Subcontractor Shall be reimbursed at one hundred percent (100%) of the prevailing AHCCCS FQHC AND RHC PPS Rate. This Amendment is effective as of April 1, 2015. Retroactive is due to processing time. All other terms and conditions that have not previously been amended shall remain in full force and effect.
