24. AMENDMENT TO SOLE SOURCE CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES, INC. FOR DENTAL SERVICES TO HOMELESS INDIVIDUALS. Approve Amendment No. 2 to the Sole Source Contract between Maricopa County through its Department of Public Health Healthcare for the Homeless (HCH) Program and Central Arizona Shelter Services, Inc., (CASS) for CASS to provide dental services to homeless individuals. This amendment makes changes to Section I of General Provisions and increases the contract amount by $50,000 for budget period November 1, 2008 through October 31, 2009. This brings the contract to a new aggregate not-to-exceed amount of $150,000 for the term November 1, 2006 through October 31, 2009. The sole source contract is in compliance with the Maricopa County Procurement Code, Section MCI-344. All other term and conditions of the original Agreement shall remain in full force and effect. Funding for this contract is provided by a grant from the Health Resources and Services Administration, and will not affect the County general fund. (C-86-06-064-1-02)

AMENDMENT TO CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES FOR HOMELESS DENTAL SERVICES Approve Amendment No. 3 to sole source Contract between Maricopa County through its Health Care for the Homeless (HCH) Program and Central Arizona Shelter Services, Inc., (CASS) to extend the term of the contract from November 1, 2006 through October 31, 2010 and increase the contract by $75,000 for CASS to provide dental services to homeless individuals. This amendment brings the contract to a new not-to-exceed amount of $225,000 for the term November 1, 2009 through October 31, 2010. All other term and conditions of the original Agreement shall remain in full force and effect. The sole source contract is in compliance with the Maricopa County Procurement Code, Section MCI-344. 22. AMENDMENT TO CONTRACT WITH CENTRAL ARIZONA SHELTER SERVICES INC. FOR DENTAL SERVICES TO THE HOMELESS Approve Amendment No. 4 to sole source Contract between Maricopa County through its Health Care for the Homeless (HCH) Program and Central Arizona Shelter Services, Inc., (CASS) to provide dental services to homeless individuals. This amendment changes the Contract Amount from an aggregate not-to-exceed total for $225,000 to $300,000 and provides new funding in the amount not-to-exceed $75,000 for the budget period November 1, 2010 through October 31, 2011. This amendment also extends the Expiration Date through October 31, 2011. This amendment also makes administrative changes to the contract. All other terms and conditions of the original contract shall remain in full force and effect. The sole source contract is in compliance with the Maricopa County Procurement Code, Section MCI-344.
