48. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR HIV PREVENTION SURVEILLANCE PROGRAM Approve Amendment No. 2 to Intergovernmental Agreement (IGA) No. HG652198 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health to provide grant funding to the HIV Prevention/Surveillance Program. The amendment replaces the original price sheet with a new price sheet in the amount not to exceed $746,982.50 for the budget period effective from January 1, 2009 through December 31, 2009. The term of this IGA is January 1, 2006 through December 31, 2011. The Department of Public Health indirect rate for FY 2008-09 is 18%. This grant deviates from County policy A2505 and does not allow for full indirect cost reimbursement. The grant indirect costs are reimbursable at a rate of 10%. Full Indirect costs are estimated at $122,233.50, of which $67,907.50 is recoverable and $54,326 is unrecoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funds for this IGA are provided by a grant from ADHS and does not affect the County’s general fund. AMENDMENT TO IGA FROM ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIV PREVENTION SERVICES Approve Amendment No. 3 to Intergovernmental Agreement (IGA) No. HG652198 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide grant funding for the HIV Prevention Services. This amendment is for the budget period from January 1, 2010 to December 31, 2011, bringing the budget amount not-to-exceed $645,808. The term is January 1, 2006 to December 31, 2011. This grant deviates from County policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 10% indirect cost reimbursement. The Department of Public Health’s indirect rate for FY 2009-10 is 19.5%. Total indirect expenses are estimated to be $114,484, of which $58,710 is recoverable and $55,774 is not recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. AMENDMENT TO IGA FROM ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIV PREVENTION SERVICES Approve Amendment No. 4 to Intergovernmental Agreement (IGA) No. HG652198 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide grant funding for the HIV Prevention Services. This amendment is for the budget period from January 1, 2011 to December 31, 2011, and increases the budget by $268,299 for a not-to-exceed total of $914,107. The term is January 1, 2006 to December 31, 2011. This grant deviates from County policy A2505 and does not allow for full indirect cost reimbursement, but a maximum of 10% indirect cost reimbursement. The Department of Public Health’s indirect rate for FY 2010-2011 is 17%. Total indirect expenses are estimated to be $141,271, of which $83,102 is recoverable and $58,169 is not recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
