A-3. AMENDMENT TO INTERGOVERNMENTAL AGREEMENTS WITH SPECIAL HEALTH CARE DISTRICT AND COMMUNICATION WITH HRSA REGARDING RYAN WHITE HIV/AIDS PROGRAM Approve amendment No. 1 to the following described Intergovernmental Agreements (IGA) between Maricopa County Special Health Care District (SHCD) d.b.a. Maricopa Integrated Health Services (MIHS) and Maricopa County through the Workforce Management and Development Department: C-86-06-831-2 (for Primary Care Services) C-86-06-844-2 (for Mental Health Services) C-86-06-845-2 (for Substance Abuse Treatment Services), in the not-to-exceed amount of $5,500,000 for FY 2007-08 - FY 2009-10. This Amendment is funded by a grant from Health Resources and Services Administration (HRSA), (1 H3MHA 08443-01) approved by the Board on July 12, 2006. The purpose of this IGA Amendment is to authorize MIHS to provide primary care services, mental health services, and substance abuse treatment services to qualified Ryan White HIV/AIDS Program Part A recipients and to replace the use of Exhibit 30 regarding payment for services in the master IGA between Maricopa County and the Special Health Care District, date November 1, 2004. This Amendment is effective from March 1, 2007 until February 28, 2010. In addition, the purpose of the action is to consolidate the three existing IGAs under C-31-10-001-3-00. Also, authorize the Chairman of the Board of Supervisors, on behalf of the Board, to send a letter to Dr. Hopson, Director, HIV/AIDS Bureau, Health Resources and Services Administration (HRSA), discussing options to resolve reimbursement rates to Maricopa Integrated Health System, under the Ryan White Program Part A, as discussed in Executive Session on August 17, 2009. 9. AMENDMENT TO IGA BETWEEN MARICOPA COUNTY THROUGH THE RYAN WHITE PART A PROGRAM AND MARICOPA INTEGRATED HEALTH SYSTEM Approve Amendment No. 3 to Intergovernmental Agreement Nos. C-86-06-831-2-00 (for Primary Care Services), C-86-06-844-2-00 (for Mental Health Services), and C-86-06-845-2-00 (for Substance Abuse Treatment Services) and their related Amendments #1 (C-31-10-001-3-00) and #2 (C-31-10-012-3-00) (collectively the “Agreement”) between Maricopa Integrated Health Systems and Maricopa County through the Ryan White Part A Program. The purpose of this amendment is to allow for the provision of Medical Case Management and Early Intervention Services for Ryan White Part A clients. 21. AMENDMENT TO IGA BETWEEN MARICOPA COUNTY THROUGH THE RYAN WHITE PART A PROGRAM AND MARICOPA INTEGRATED HEALTH SYSTEM Approve Amendment No. 4 to extend previously approved Intergovernmental Agreements Nos. C-86-06-831-2-00 (for Primary Care Services), C-86-06-844-2-00 (for Mental Health Services), and C-86-06-845-2-00 (for Substance Abuse Treatment Services) and their related Amendments #1 (C-31-10-001-3-00) and #2 (C-31-10-012-3-00) and #3 (C-31-10-012-3-01) (collectively the “Agreement”) between Maricopa Integrated Health Systems and Maricopa County through the Ryan White Part A Program through February 29, 2012. The purpose of this amendment is to continue providing Core Medical and Support Services to Ryan White Part A clients in Ryan White Part A Fiscal Year 2011 (March 1, 2011 – February 29, 2012). The expenditure limit for FY 2011 is unchanged at $833,333. The FY 2012 expenditure limit is $1,166,667. (C-31-10-012-3-02)

15. AMDENDMENT TO IGA BETWEEN MARICOPA COUNTY THROUGH THE RYAN WHITE PART A PROGRAM AND MARICOPA INTEGRATED HEALTH SYSTEM Approve Amendment No. 5 to Intergovernmental Agreement Nos. C-86-06-831-2-00 (for Primary Care Services), C-86-06-844-2-00 (for Mental Health Services), and C-86-06-845-2-00 (for Substance Abuse Treatment Services) and their related Amendments #I (C-31-10-001-3-00), #2 (C-31-10-012-3-00, #3 (C-31-10-012-3-01) and #4 (C-31-10-012-3-02 (collectively the "Agreement") between Maricopa County by and through the Ryan White Program, a division of the Business Strategies and Health Care Programs Department (referred to collectively as "the County") and Maricopa County Special Health Care District d/b/a Maricopa Integrated Health System ("MIHS"). The purpose of this amendment is to provide Oral Health Services, including Direct Dental Services and Dental Coordination Services in an amount not to exceed $500,000 with an effective date of November 2, 2011.
