37. AMENDMENT TO LEASE AGREEMENT WITH THOMAS VERDRES DBA T&R PROPERTIES FOR HEALTHY START CLINIC SPACE Approve and execute Amendment No. 1 to Amendment of Lease Agreement No. L-7389 with Thomas Vedres DBA T&R Properties for office and clinic space at 2737 W. Southern Avenue, Tempe, AZ. Maricopa County will lease an additional 850 square feet adjacent office space at a flat rate of $750.00 per month making the total leased space at 3,702 square foot. The lease term will be amended to commence on November 1, 2009 and terminate October 30, 2012. All other terms and conditions of the original Lease Agreement remain the same and in full force and effect This item was indorsed by the County Review Committee during its October 2009 Committee meeting. Tenant improvements will be at no cost to the County. (C-86-07-005-4-01)

80. AMENDMENT TO LEASE AGREEMENT WITH THOMAS VEDRES DBA T&R PROPERTIES FOR HEALTHY START Approve and authorize the Chairman of the Board to execute amendment No. 2 of Lease Agreement No. L-7389 with Thomas Vedres DBA T&R Properties, LLC for 3,703 s/f office and clinic space at 2737 W. Southern Ave., Tempe, AZ. The lease term shall be extended five additional years and the Lessor shall replace carpeting with VTC to match the kitchen flooring as directed at Exhibit “A” and “B” of the lease amendment. The five (5) year Agreement shall commence on November 1, 2012 and terminate October 31, 2017. The rental rate for the first year is $14.44 s/f or an annual cost of $53,456.88 plus rental tax and with a 3 percent increase each year for the remaining four (4) years. The rental rate for year two is $14.87 s/f or annual cost of $$55,048.74, year three is $15.32 s/f or an annual cost of $56,714.64, year four is $15.78 s/f or annual cost of $58,417.56 and year five is $16.25 s/f or annual cost of $60,170.03.
