39. AMENDMENT TO IGA WITH THE MESA FIRE DEPARTMENT FOR CHILDHOOD IMMUNIZATIONS Approve Amendment No. 1 to Intergovernmental Agreement (IGA) between the City of Mesa through the Mesa Fire Department (Mesa Fire) and Maricopa County through the Department of Public Health’s Community Health Nursing Program (CHN) for immunizations to school aged children. Amendment No. 1 extends the term of the IGA beginning June 1, 2010 through May 1, 2011, for a total funding amount of $75,000. This amendment also makes administrative changes to the IGA. All other terms and conditions of the original IGA shall remain in full force and effect. Funding for this IGA is provided by a grant from Arizona Department Health Services, and will not affect the County general fund. (C-86-07-058-2-01)

A-5. AMENDMENT TO THE IGA WITH THE MESA FIRE DEPARTMENT FOR IMMUNIZATIONS Approve Amendment No. 2 to Intergovernmental Agreement (IGA) between the City of Mesa through the Mesa Fire Department (Mesa Fire) and Maricopa County through the Department of Public Health’s Community Health Nursing Program (CHN) for immunizations to school aged children. Amendment No. 2 adds terms and conditions pursuant to the administration of the H1N1 immunization to the community. This amendment adds services pertaining to providing funding pursuant to H1N1 immunization activities for an amount not-to-exceed $136,000 for the term ending July 30, 2010. This increases the contract not-to-exceed amount to $211,000 for the term of the contract ending May 1, 2011. This amendment also makes administrative changes to the IGA. All other terms and conditions of the original IGA shall remain in full force and effect. Funding for this IGA is provided by a grant from Arizona Department Health Services (CFDA 93.069), and will not affect the County general fund. Note: *Budget is NO only because FY11 budget has not been adopted, however, funding for this contract is available in FY11 budget submission. *Services pertaining to Childhood Services will continue to be paid out of a grant from ADHS (CFDA 93.268). Amendment 3 also increases the contract by $75,000 for the budget period June 3, 2011 through May 31, 2012, for a new not-to-exceed Contract total of $511,000. All other terms and conditions of the original IGA shall remain in full force and effect. 76. CLAIM FOR PAYMENT TO CITY OF MESA, FIRE AND MEDICAL DEPARTMENT FOR CHILDHOOD IMMUNIZATION SERVICES Approve Claim for Payment of unpaid invoice under Intergovernmental Agreement, Amendment no. 4, (C-86-07-058-2-04) between the City of Mesa, Mesa Fire Department (MFD) and Maricopa County by and through its Department of Public Health, Community Health Nursing (CHN) for childhood immunizations services provided by MFD. The unpaid invoice number is (05-13), dated May 30, 2013, in the amount of $75,000.00 for services rendered. Per MFD, the invoice was sent in within the timelines of the terms and conditions of the contract to the MCDPH, CHN. The IGA Amendment No. 4, budget period is June 3, 2012 to May 31, 2013. PH finance has reconciled the payments within that budget period and no payments were made to The City of Mesa, Mesa Fire Department. (C-86-07-058-2-05)
