19. AMENDMENT TO IGA WITH THE AVONDALE FIRE DEPARTMENT FOR CHILDHOOD IMMUNIZATIONS Approve Amendment No. 1 to the Intergovernmental Agreement (IGA) by and between The City of Avondale by and through The Avondale Fire Department (Avondale Fire) and Maricopa County through its Department of Public Health’s Community Health Nursing Program (CHN). This amendment will delete and replace the Expiration Date, February 31, 2011 with February 28, 2011 on the Coversheet, Line 6. This amendment also makes additional administrative changes to the Section I, General Provisions. Funding for this IGA is provided by a grant from Arizona Department Health Services, and will not affect the County general fund. (C-86-08-045-2-01)

58. AMENDMENT TO IGA WITH CITY OF AVONDALE FOR CHILDHOOD IMMUNIZATIONS Approve Amendment No. 2 to Intergovernmental Agreement (IGA) between the City of Avondale through the Avondale Fire Department and Maricopa County through the Department of Public Health for immunizations to school aged children to extend the expiration date to February 29, 2012, and to increase the funding by $75,000 for the budget period February 28, 2011 to February 29, 2012, for a new not-to-exceed amount $300,000. All other terms and conditions of the original IGA shall remain in full force and effect. Funding for this IGA is provided by a grant from Arizona Department Health Services, and will not affect the County general fund. (C-86-08-045-2-02)
