33. AMENDMENT TO LEASE AGREEMENT WITH ST MARYS FOOD BANK FOR WIC CLINC SPACE Approve and execute amendment No. 1 to Lease Agreement No. L-7403 with St. Mary’s Food Bank located at 3003 W. Thomas Road, Phoenix, AZ. Lessee occupies 4,488 square feet of office/clinic space for Women and Infant Children services. The rental rate is reduced from $12.00 per square foot or an annual rate of $53,856 to $11.25 per square foot or an annual rate of $50,490. The new rental rate will commence upon approval by the Board of Supervisors and will terminate on October 30, 2012. The lease contains a 90-day termination provision and a six-month hold-over provision. All other terms and conditions of the original lease remain in full force and effect. Lessor will discontinue providing janitorial services to the leased facility and now Lessor and Lessee mutually desire to amend the lease to adjust the annual rent schedule to reflect the decrease in services provided by Lessor. The County Facility Review Committee endorsed this amendment at their October meeting. (C-86-08-046-4-01)

34. ADMINISTRATIVE CORRECTION TO LEASE AGREEMENT WITH ST MARY'S FOOD BANK Approve an administrative correction to the action taken by the Board on November 18, 2009, under C-86-08-046-4-01, for Lease No. L-7403 with St Mary’s Food Bank as Lessor, for office/clinic space for Women and Infant Care services. 36. AMENDMENT TO LEASE WITH THE ST MARY’S FOOD BANK ALLIANCE FOR WIC CLINIC Approve and execute the Second Amendment to Lease Agreement No. L-7403 with St. Mary’s Food Bank Alliance for WIC Clinic and office space located at 3003 W. Thomas Road, Phoenix, AZ. The lease term shall be amended to commence on the first day of the month following Board approval and terminate at the previously approved date of October 30, 2013. The Lessee agrees to pay as base rent during the remaining months of the current lease agreement $11.25 s/f or an annual payment of $72,000.00 plus rental tax. The square feet of leased spaced shall be increased by 1,912 s/f making an amended leased area of 6,400 s/f. Lessor shall provide 11 additional parking spaces to total 36 spaces. All other terms and conditional of the current Lease Agreement shall remain in full force and effect. The County Facility Review Committee indorsed amending Lease Agreement No. L-7403. 45. AMENDMENT TO LEASE L-7403 WITH ST MARY’S FOOD BANK ALLIANCE, INC FOR WIC CLINIC Approve and authorize the Chairman to execute, Amendment No. 3 to Lease Agreement No. L-7403 with St. Mary’s Food Bank Alliance, Inc., an Arizona non-profit corporation to extend the term of the lease agreement for 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. Amendment No. 3 will extend the term of the existing lease for sixty months. The new sub-lease term will begin October 31, 2013 and expire October 31, 2018, unless terminated earlier as provided for in the agreement. The monthly base rent during the lease term will remain the same at $6,000.00 per month plus rental tax ($11.25 per square foot). This lease amendment was endorsed by the County Facility Review Committee during September 2013. (C-86-08-046-4-04)

RESCIND AND APPROVE AMENDMENT TO LEASE L-7403 WITH ST MARY’S FOOD BANK ALLIANCE, INC FOR WIC CLINIC Rescind the action taken by the Board on October 23, 2013 under item (C-86-046-6-04) Amendment No. 3 to Lease Agreement No. L-7403 with St. Mary’s Food Bank Alliance, Inc., an Arizona non-profit corporation to extend the term of the lease agreement for 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. Amendment No. 3 will extend the term of the existing lease for sixty months. The lease agreement was not signed and terms have been renegotiated. Also, approve and authorize the Chairman to execute, Amendment No. 3 to Lease Agreement No. L-7403 with St. Mary’s Food Bank Alliance, Inc., an Arizona non-profit corporation to extend the term of the lease agreement for 6,400 square feet of office and clinic space at 3003 West Thomas Road, Phoenix. Amendment No. 3 will extend the term of the existing lease for twelve months. The new sub-lease term will begin October 31, 2013 and expire October 31, 2014, unless terminated earlier as provided for in the agreement. The monthly base rent during the lease term will remain the same at $6,000.00 per month plus rental tax ($11.25 per square foot). This revised lease amendment was endorsed by the County Facility Review Committee during April 2014. (C-86-08-046-4-05)
