49. AMENDMENT TO CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 3 to Contract No. C-86-08-054-1, between the Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health, Office of Oral Health for the provision of dental insurance services to individuals affected by HIV/AIDS. This amendment extends the expiration date of contract to February 28, 2010. This amendment also increases the contract amount by $871,028, bringing the new total to $1,221,028 for the budget term March 1, 2009 through February 28, 2010. Bringing the aggregate total of this contract not to exceed $1,986,028 for the term March 1, 2008 through February 28, 2010. All other terms and conditions of the original contract shall remain in full force and effect. Funding for this contract is provided by a grant from the federal Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program and will not affect the County general fund. Delta Dental is a successful respondent to a Request For Proposals (08-007) issued by the Department of Public Health on January 2, 2008. This is to reflect total award to the sub-recipient only. AMENDMENT TO CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 4 to Contract No. C-86-08-054-1, between the Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health, Office of Oral Health for the provision of dental insurance services to individuals affected by HIV/AIDS. Amendment No. 4 increases the contract amount by $72,000 bringing the new total to $1,293,028 for the budget term March 1, 2009 through February 28, 2010, for an aggregate total of not-to-exceed $2,058,028. All other terms and conditions of the original contract shall remain in full force and effect. Funding for this contract is provided by a grant from the Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program and will not affect the County general fund. Amendment No. 5 extends the budget period beginning March 1, 2010 through February 28, 2011 in the amount of $1,229,788. This brings the aggregate total from $2,058,028 to new not-to-exceed amount of $3,287,816. All other terms and conditions of the original contract shall remain in full force and effect. Funding for this contract is provided by a grant from the Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County General Government Department, Ryan White Part A Program and will not affect the County general fund. The amount of $1,229,788, is the FY 2010-11 total allocation to the subrecipient. AMENDMENT TO CONTRACT WITH DELTA DENTAL PLAN OF ARIZONA FOR DENTAL INSURANCE SERVICES Approve Amendment No. 6 to Contract No. C-86-08-054-1, between the Delta Dental Plan of Arizona and Maricopa County through its Department of Public Health, Office of Oral Health for the provision of dental insurance services to individuals affected by HIV/AIDS. This amendment changes the Contract Amount from an aggregate not-to-exceed total for $3,287,816 to $3,530,816. This amendment also increases the funding by $243,000, bringing the budget total from $1,229,788 to $1,472,788 for the budget period March 1, 2010 through February 28, 2011. All other terms and conditions of the original contract shall remain in full force and effect. Funding for this contract is provided by a grant from the Department of Health and Human Services, Health Resources and Services Administration (HRSA), the Ryan White (RW) Comprehensive HIV/AIDS Treatment Modernization Acts of 2006 and the Maricopa County Workforce Management Development, Ryan White Part A Program and will not affect the County general fund. LIMITED WAIVER OF CONFLICT OF INTEREST FOR GREGORY HARRIS OF LEWIS & ROCA Approve a request for a limited waiver of conflict of interest for Gregory Harris of the law firm of Lewis & Roca, for the limited purpose of the following two (2) items: (1) providing legal advice and counsel to Delta Dental, Inc in negotiating a fixed-price contract for dental screening and fluoride varnish, and (2) providing legal advice and counsel to Delta Dental in interpreting the provisions regarding release of patient information within an existing contract (C-86-08-054-1), which provides insurance services to persons affected by HIV/AIDS, and is funded by the Ryan White Program, Part A .
