Amendment No. 1 revises the Price Sheet to reflect an increase in the rate per visit, and makes administrative changes to the contract. The budget period is January 1, 2009 through December 31, 2009. The term for this IGA is January 1, 2008 through December 31, 2012. All other terms and conditions of the original IGA shall remain in full force and effect. This Procurement Change Order increases funding by $780,351 for a total funding amount of $1,803,956 for budget period January 1, 2009 through December 31, 2009. The term for this IGA is January 1, 2008 through December 31, 2012. All other terms and conditions of the original IGA shall remain in full force and effect. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. This amendment is for the grant cycle that started January 1, 2009 with the indirect rate of 18%.
