33. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR TUBERCULOSIS CONTROL Approve Amendment No. 1 to the Intergovernmental Agreement (IGA) No. HG852310 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health to provide additional grant funding for Tuberculosis Control. This amendment budget amount is $280,845 for the budget period of January 1, 2009 through December 31, 2009. This amendment also makes administrative changes to the SPECIAL TERMS AND CONDITIONS. The Department of Public Health’s indirect rate for FY 2008-09 is 18%. Indirect costs are estimated at $42,841and are fully recoverable. Half of the grant award is already included in the FY 2008-09 adopted budget in the amount of $140,423 An appropriation adjustment is not requested at this time as these funds were included in the FY 2008-09 Adopted Budget. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR TUBERCULOSIS CONTROL Approve Amendment No. 2 to IGA Contract (HG852310) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health for grant funding the Tuberculosis Control Program (TB). The amendment replaces the price sheet with a total budget amount of $339,826, for the budget period from January 1, 2010 through December 31, 2010. The term of this IGA is January 1, 2008 through December 31, 2012. All other provisions of this agreement remain unchanged. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. Indirect costs are estimated at $55,453 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. This amendment increases the current budget amount by $6,500, bringing the total not-to-exceed amount from $339,826 to $346,326, for the budget period January 1, 2010 through December 31, 2010. The term of this IGA is January 1, 2008 through December 31, 2012. All other provisions of this agreement remain unchanged. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. Indirect costs are estimated at $1,060 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. RESCIND AND APPROVE AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR TUBERCULOSIS CONTROL PROGRAM Rescind action taken by the Board of Supervisors on December 1, 2010 (C-86-08-062-2-03) to approve and execute Amendment No. 3 to IGA Contract HG852310 Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide additional grant funding for the Tuberculosis Control Program (TB), for the amount of $6,500. This action is rescinded due to an incorrect amendment document forwarded by ADHS to MCDPH. Also approve revised Amendment No. 3 to Intergovernmental Agreement (IGA) Contract (HG852310) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) to provide additional grant funding for the Tuberculosis Control Program (TB). This amendment increases the current budget amount by $31,500, bringing the total not-to-exceed amount from $339,826 to $371,326, for the budget period January 1, 2010 through December 31, 2010. The term of this IGA is January 1, 2008 through December 31, 2012. All other provisions of this agreement remain unchanged. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. Indirect costs are estimated at $5,140 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. The Price Sheet of Amendment No. 3 is replaced by revised Price Sheet, Amendment No. 4. The Price Sheet amount is not-to-exceed $339,826, effective January 1, 2011 through December 31, 2011. The term of this IGA is January 1, 2008 through December 31, 2012. All other provisions of this agreement remain unchanged. The Department of Public Health indirect rate for FY 2010-11 is 17%. Indirect costs are estimated at $49,376 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
