23. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR STD SERVICES Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) No. HG854321 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide grant funding for STD Services. This amendment budget amount is not-to-exceed $370,748 for the budget term January 1, 2009 through December 31, 2009. This amendment also makes administrative changes to the Uniform Terms and Conditions. All other terms and conditions of the original IGA remain unchanged and in full force and effect. The Department of Public Health’s indirect rate for FY 2008-09 is 18%. Grant indirect expenses are fully recoverable and are estimated at $56,555. Funds for this IGA are provided by a grant from ADHS and does not affect the County’s general fund. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. This amendment replaces the original price sheet for prior year, January 1, 2008 through December 31, 2008 and the budget term is decreased by $1,865. (C-86-08-064-2-03)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR STD SERVICES Approve Amendment No. 4 to the Intergovernmental Agreement (IGA)( HG854321) between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide grant funding for STD Services. This amendment replaces the Price Sheet for Amendment No. 2 for a budget amount not-to-exceed $358,917. The budget period is from January 1, 2010 through December 31, 2010. All other provisions shall remain unchanged. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. Indirect costs are estimated at $58,568 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. AMENDMENT TO IGA WITH ARZONA DEPARTMENT OF HEALTH SERVICES FOR STD SERVICES Approve Amendment No. 6 to the Intergovernmental Agreement (IGA) Contract No. HG854321 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide grant funding for STD Services. The Price Sheet of Amendment No. 4 is replaced by revised Price Sheet for Amendment No. 6. The Price Sheet amount is not-to-exceed $358,917, effective January 1, 2011 through December 31, 2011. Also, this amendment includes additional administrative changes. All other provisions shall remain unchanged. The Department of Public Health indirect rate for FY2010-11 is 17%. Indirect costs are estimated at $52,150 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. This amendment is effective upon final signature, pursuant to Special Terms and Conditions, Page Ten (10), Provision Two (2), Contract Extension (4 Years), this Contract is extended, for the final year, through December 31, 2012.
