35. AMENDMENT TO FACILITY USE AGREEMENT FOR CHANDLER WIC CLINIC Approve Amendment No. 1 to the full service Facilities Use Agreement No. P50108 with Catholic Healthcare West, a California non-profit public benefit Corporation d.b.a. Chandler Regional Medical Center , Licensee, for the use of clinic space to perform vaccinations to uninsured children at the WIC clinic located at 3002 N. Arizona Ave. , Chandler , AZ. The term of the Agreement is from January 1, 2009 through December 31, 2009. License agrees to provide the services at no cost to the County and the recipients. 44. AMENDMENT TO FACILITY USE AGREEMENT WITH CATHOLIC HEALTHCARE WEST DBA CHANDLER REGIONAL MEDICAL CENTER Retroactively approve and execute Amendment No. 2 to the full service Facilities Use Agreement No. P50108 with Catholic Healthcare West, a California non-profit public benefit corporation d/b/a Chandler Regional Medical Center, Licensee, for the use of a clinic room to perform vaccinations to uninsured children at the WIC clinic located at 3002 N. Arizona Ave., Chandler, AZ. The term of the Agreement will automatically renew each year through the remaining term of the lease (March 31, 2013) unless terminated earlier by either party giving a 30-day written notice of termination. Licensee agrees to provide the Services at no cost to the County and the recipients. County agrees to provide access and promotion assistances without monetary remuneration. (C-86-08-071-0-02)

AMENDMENT TO FACILITY USE AGREEMENT WITH DIGNITY HEALTH FOR IMMUNIZATION SERVICES AT CHANDLER WIC CLINIC Approve Amendment No. 3 to the full service Facilities Use Agreement No. P50108 with Dignity Health (formerly known as Catholic Healthcare West), a California non-profit public benefit corporation d/b/a Chandler Regional Medical Center, Licensee, for the use of a clinic room to perform vaccinations to uninsured children at the WIC clinic located at 3002 N. Arizona Ave., Chandler, AZ. The term of the Agreement will extend one (1) year, commencing April 1, 2013 and ending on March 31, 2014. Unless terminated earlier by either party by giving a thirty (30) day written notice of termination, the agreement will automatically renew for one (1) additional year terminating March 31, 2015. Licensee agrees to provide the Services at no cost to the County and the recipients. County agrees to provide access and promotion assistances without monetary remuneration. (C-86-08-071-0-03)

28. AMENDMENT TO FACILITY USE AGREEMENT NO. P-50108 WITH DIGNITY HEALTH, D/B/A CHANDLER REGIONAL MEDICAL CENTER Approve, and authorize the Chairman to execute, Facility Use Agreement P-50108 between Dignity Health d/b/a/ Chandler Regional Medical Center (User) and Maricopa County by and through its Department of Public Health (MCDPH) for the use of a clinic room to perform free vaccinations to uninsured infants, children, adult children and adults at the Women, Infant and Children (WIC) clinic located at 3002 N. Arizona Avenue, Chandler, AZ. The term of the Agreement will extend one (1) year, commencing April 1, 2015 and ending on March 31, 2016. Unless terminated earlier by either party by giving a thirty (30) day written notice of termination, the agreement will automatically renew for one (1) additional year terminating March 31, 2017. This Facility Use Agreement allows for the occasional use of office space as indicated in Exhibit A at the Chandler WIC facility by User to provide immunization to uninsured recipients at no cost or remuneration to the County. The Agreement expands the definition of “Recipients” and “Services” to include infants, flu vaccinations to adults and children and vaccine at hospital cost, and discounts to students enrolled in a school program leading to a healthcare degree, and include adults with decreased accessibility or financial acumen to obtain vaccinations and to adult students seeking required vaccines to complete their healthcare degree. AMENDMENT TO FACILITY USE AGREEMENT WITH DIGNITY HEALTH, D/B/A CHANDLER REGIONAL MEDICAL CENTER FOR USE OF SPACE Approve and authorize the Chairman to execute Amendment No. 5 to Facility Use Agreement P-50108 with Dignity Health d/b/a/ Chandler Regional Medical Center (User) for the use of a clinic room to perform vaccinations to uninsured children at the PHSX-WIC clinic located at 3002 N. Arizona Avenue, Chandler, AZ. This will generally occur the first Thursday of each month, from 9 am to 11 am, per the attached Exhibit A. This Facility Use Agreement allows for use of office space at the Chandler WIC facility by Dignity Health to provide immunization to uninsured recipients at no cost or remuneration to the County. The term of the Agreement will extend one (1) year, commencing April 1, 2017 and expiring on March 31, 2018, with one (1) automatic one (1) year renewal. Either Party may terminate the Agreement with 30-days written notice to the other. Minor changes to the amendment are made to facilitate proper administration of the Agreement. (C-86-08-071-0-05)
