57. AMENDMENT TO CONTRACT WITH BANNER HEALTH SYSTEMS D/B/A BANNER GOOD SAMARITAN MEDICAL CENTER FOR PUBLIC INFORMATION LINE Approve Amendment No. 1 to Contract between Banner Health System d/b/a Banner Good Samaritan Medical Center (BGSMC) and Maricopa County through the Department of Public Health, Office of Preparedness and Response (OPR) formerly known as Public Health Emergency Management Program (PHEM) to provide a Public Information Line (PIL). BGSMC will provide a PIL hotline that will operate 24-hours a day, seven days a week. The contract term is from July 1, 2009 through June 30, 2010 for the budget amount not-to-exceed $134,000. Maricopa County has the right to extend this contract for additional periods, not-to-exceed a total term of four years. (C-86-09-011-3-01)

33. RESCIND AND APPROVE AMENDMENT WITH BANNER HEALTH D/B/A BANNER GOOD SAMARITAN MEDICAL CENTER FOR PUBLIC INFORMATION LINE 1. Rescind the action taken on July 22, 2009 (C-86-09-011-3-01) to approve Amendment No. 1 for the Banner Health System d/b/a Banner Good Samaritan Medical Center (BGSMC) to provide a Public Information Line (PIL). BGSMC will provide a PIL hotline that will operate 24-hours a day, 7 days a week. This Amendment No. 1 was never executed and BGSMC requested an updated version of the Amendment No. 1 to be approved. 2. Approve (revised) Amendment No. 1 to Contract between Banner Health System d/b/a Banner Good Samaritan Medical Center (BGSMC) and Maricopa County through the Department of Public Health, Office of Preparedness and Response (OPR) to provide a Public Information Line (PIL). BGSMC will provide a PIL hotline that will operate 24-hours a day, 7 days a week. The contract term is from July 1, 2009 through June 30, 2010 for the budget amount is not-to-exceed $134,000. Maricopa County has the right to extend this contract for additional periods, not-to-exceed a total term of four years. (C-86-09-011-3-02)
