AMENDMENT TO CONTRACT WITH PHOENIX CHILDREN’S HOSPITAL FOR MOBILE MEDICAL CLINIC Approve Amendment No. 1 to Contract between Phoenix Children’s Hospital (PCH) and Maricopa County through its Department of Public Health, Healthcare for the Homeless (HCH) Program to provide medical services to homeless youth via a mobile medical clinic. This amendment makes administrative changes to Section III. Work Statement. All other terms and conditions of the original contract shall remain in full force and effect including the term of November 1, 2008 to October 31, 2009. This subcontract is exempt from the Maricopa County Procurement Code under Section MC1-102C, as it is a prior designated subcontractor of the Grantor. AMENDMENT TO CONTRACT WITH PHOENIX CHILDRENS HOSPITAL FOR MOBILE MEDICAL CLINIC Approve Amendment No. 2 to Contract between Phoenix Children’s Hospital (PCH) and Maricopa County through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) Program and to provide medical services to homeless youth via a mobile medical clinic. This amendment increases the funding by $160,000, bringing the total contract amount not-to-exceed from $133,762 to $293,762, for the budget period November 1, 2009 to October 31, 2010. The budget term is November 1, 2008 to October 31, 2010. This amendment also makes administrative changes to the contract. All other terms and conditions of the original contract shall remain in full force and effect. This subcontract is exempt from the Maricopa County Procurement Code under Section MC1-102C, as it is a prior designated subcontractor of the Grantor. RESCIND AND APPROVE AMENDMENT TO CONTRACT WITH PHOENIX CHILDREN’S HOSPITAL FOR YOUTH HOMELESS SERVICES Rescind action taken on November 16, 2011, C-86-09-036-3-04, to approve Amendment No. 4 to Contract between Phoenix Children’s Hospital (PCH) and Maricopa County through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) Program and to provide medical services to homeless youth via a mobile medical clinic. Phoenix Children’s Hospital has not signed Amendment No. 4 and requested an update of terms. Also, Approve Amendment No. 4 to Contract between Phoenix Children’s Hospital (PCH) and Maricopa County through its Department of Public Health (MCDPH), Healthcare for the Homeless (HCH) Program and to provide medical services to homeless youth via a mobile medical clinic.
