36. IGA WITH ADHS FOR BREASTFEEDING PEER COUNSELING PROGRAM Approve Intergovernmental Agreement (IGA) No. HG961186 between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health to provide grant funding for the Breastfeeding Peer Counseling Program. The term for this IGA is October 1, 2008 through September 30, 2013. The budget amount of $731,870 is for the budget period of October 1, 2008 through September 30, 2009. The Department of Public Health’s indirect rate for FY 2008-09 is 18%. Indirect costs are estimated at $111,641 and are fully recoverable. Also approve revenue and expenditure appropriation adjustments to the Public Health Grant Fund (Department 860, Fund 532, Function Code 0000) associated with the aforementioned grant in an amount of $370,847 for FY 2008-09. The appropriations adjustment is necessary because these funds were not included in the FY 2008-09 budget. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditures of these revenues are not prohibited by the budget law. The approval of this budget adjustment does not alter the budget constraining the expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. Of the total grant award, $178,056 was already included in the FY 2008-09 adopted budget. Funds for this IGA are provided by a grant from ADHS and do not increase the County’s general fund. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR BREASTFEEDING PEER COUNSELING PROGRAM Approve Amendment No. 1, Intergovernmental Agreement (IGA) No. HG961186 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide increased grant funding for the Breastfeeding Peer Counseling Program. This amendment replaces Price Sheet of original contract with a revised Price Sheet for an increased amount of $324,598, bringing the new total not to exceed $1,056,468. The revised Price Sheet is effective July 1, 2009 through September 30, 2009. The term of this IGA is October 1, 2008 through September 30, 2013. The Department of Public Health’s indirect rate for FY 2008-09 is 18%. Indirect costs are estimated at $49,515 and are fully recoverable. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funds for this IGA are provided by ADHS and do not affect the County’s general fund. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICE FOR BREASTFEEDING PEER COUNSELING PROGRAM Approve Amendment No. 2, Intergovernmental Agreement (IGA) No. HG961186 between Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health to provide increased grant funding for the Breastfeeding Peer Counseling Program. This amendment replaces Price Sheet of original contract with a revised Price Sheet for not-to-exceed $1,082,363. The revised Price Sheet is effective October 1, 2009 through September 30, 2010. The term of this IGA is October 1, 2008 through September 30, 2013. The Department of Public Health’s indirect rate for FY 2009-10 at 19.5%. Indirect costs are estimated at $176,620 and are fully recoverable. All other terms and provisions of the original Agreement are not modified and as set forth herein shall remain in full force and effect. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. The revised budget is over by $4,282 and will be reduced in the next budget appropriation adjustment. Funds for this IGA are provided by ADHS and do not affect the County’s general fund.
