RESIDENT ROTATION AGREEMENT WITH MAYO CLINIC FOR LEARNING EXPERIENCES Approve the Resident Rotation Agreement and Addendum as Attachment A with Mayo Clinic through its Mayo School of Graduate Medical Education to allow residents to participate in learning experiences at the Maricopa County Department of Public Health (MCDPH). 30. TERMINATE THE RESIDENT ROTATION AGREEMENT WITH MAYO CLINIC FOR THE FAMILY MEDICINE RESIDENCY PROGRAM Terminate the Affiliation Agreement (Family Medicine Residency Program) with the Mayo Clinic through its Mayo School of Graduate Medical Education to provide a residency training program for Resident Physicians in training. (C-86-09-044-3-00) Pursuant to Paragraph 5.2 (a) of the Agreement, “Either party to this Agreement may terminate this Agreement for any reason at any time by giving at least ninety (90) day advance written notice to the other party”. The term of the original agreement was October 1, 2008 through June 30, 2013. Termination is effective March 23, 2011. (C-86-11-048-3-00)
