24. AMENDMENT TO GRANT CONTRACT FOR THE WELL WOMAN HEALTHCHECK PROGRAM CYCLE TO CARE TREATMENT PROJECT Approve Amendment No.1 to Grant Contract award from the Phoenix Affiliate of the Susan G. Komen Breast Cancer Foundation, Inc. d.b.a. the Phoenix Affiliate of Susan G. Komen for the Cure to Maricopa County through its Department of Public Health to provide the Well Woman Healthcheck Program Cycle to Care Treatment Project. This amendment increases the contract amount by $25,000 for a total funding amount of $150,000 for budget term April 1, 2009 through March 31, 2010. The Department of Public Health’s indirect rate is 18% for FY 2008-09. Total indirect costs are estimated at $27,000 all of which is unrecoverable. The total sum of the grant consists of sub-recipient pass-through funds not subject to indirect costs. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. Funding for this grant is provided by the Susan G. Komen Breast Cancer Foundation, Inc. d.b.a. the Phoenix Affiliate of the Susan G. Komen for the Cure and will not affect the County general fund. (C-86-09-052-3-01)
