IGA WITH THE ARIZONA BOARD OF REGENTS BY AND THROUGH THE UNIVERSITY OF ARIZONA FOR TRAINING DELIVERY Approve the Intergovernmental Agreement (IGA) with The University of Arizona, Board of Regents and Maricopa County through its Department of Public Health (MCDPH) Public Health Emergency Management Program (PHEM) for training delivery. This IGA is in the amount not-to-exceed $55,000 and will be effective upon execution by both parties until August 9, 2009. AMENDMENT TO IGA WITH THE UNIVERISTY OF ARIZONA, BOARD OF REGENTS FOR TRAINING DELIVERY Approve Amendment No. 2 to the Intergovernmental Agreement (IGA) with The University of Arizona, Board of Regents and Maricopa County through its Department of Public Health (MCDPH) Office of Preparedness and Response (OPR) formally known as Public Health Emergency Management Program (PHEM) for training delivery. Amendment No. 2 exercises the option in General Provisions, Section I, 24 RIGHT TO EXTEND CONTRACT, to extend the contract for an additional period of one year from August 10, 2009 through August 9, 2010. AMENDMENT TO IGA WITH THE ARIZONA BOARD OF REGENTS, UNIVERSITY OF ARIZONA FOR TRAINING DELIVERY Approve Amendment No. 6 to the Inter-governmental Agreement (IGA) between The University of Arizona, Arizona Board of Regents and Maricopa County through its Department of Public Health (MCDPH), Office of Preparedness and Response (OPR) formally known as Public Health Emergency Management Program (PHEM) for training delivery. Amendment No. 6 exercises the option in General Provisions, Section I, Paragraph 24, Right to Extend Contract, to extend the contract for additional periods, not to exceed a total term of five (5) years. This amendment extends the expiration period to August 9, 2014. Also, this is the last year of the term. All other terms and conditions of the original agreement and its amendments shall remain in full force and effect. (C-86-09-067-3-06)

This Amendment adds to Section 3, C. Scope of Work. All other terms and conditions of the original agreement and its amendments shall remain in full force and effect.
