44. DIETETIC INTERNSHIP AGREEMENT WITH JOHN C. LINCOLN HEALTH NETWORK Approve the Affiliation Agreement (Dietetic Internship Agreement) with John C. Lincoln Health Network d.b.a. John C. Lincoln Hospital – North Mountain, John C. Lincoln Hospital – Deer Valley, and other entities of Lincoln Limited Liability Company to provide clinical nutrition training experience for graduate students in the dietetic internship. 40. TERMINATE THE AFFILIATION AGREEMENT WITH JOHN C. LINCOLN HEALTH NETWORK FOR THE DIETETIC INTERNSHIP Terminate the Affiliation Agreement (Dietetic Internship Agreement) with John C. Lincoln Health Network d.b.a. John C. Lincoln Hospital – North Mountain, John C. Lincoln Hospital – Deer Valley, and other entities of Lincoln Limited Liability Company to provide clinical nutrition training experience for graduate students in the dietetic internship. (C-86-09-078-3-00) Pursuant to Article V, Paragraph 1 of the Dietetic Internship Agreement, Maricopa County, through its Department of Public Health, wishes to terminate the above referenced agreement with the John C. Lincoln Health Network. The term of the original agreement was July 1, 2009 through June 30, 2011. Termination is effective upon approval by the Board of Supervisors. This Agreement is to be superseded by a new agreement at the request of the John C. Lincoln Health Network. (C-86-11-030-3-00)
