55. CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve Contract between Maricopa County through its Department of Public Health, Office of Family Health and Southwest Human Development (SWHD) for comprehensive community health nursing services to eligible women and their families through the South Phoenix Nurse Family Partnership Program (SPNFP). This contract provides funding to SWHD in an amount not-to-exceed $473,419 for contract period effective upon Board execution through June 30, 2010. This agreement is a subcontract to a grant from First Things First for the South Phoenix Nurse Family Partnership awarded to Maricopa County through its Department of Public Health in the amount of $1,000,000, and therefore does not increase the county general fund. Payment to contractor is on a cost reimbursement basis. Southwest Human Development was a successful respondent to a Notice of Intent issued by the Maricopa County Department of Public Health on March 3, 2009 (#PH NOI 09-026). (C-86-10-003-3-00)

32. AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve Contract Amendment between Southwest Human Development (SWHD) and Maricopa County through its Department of Public Health (MCDPH), Office of Family Health and for comprehensive community health nursing services to eligible women and their families through the South Phoenix Nurse Family Partnership Program (SPNFP). This contract amendment provides funding to SWHD in an amount not-to-exceed $556,671 for the budget period July 1, 2010 through June 30, 2011. The total aggregate contract dollar amount will increase from not-to-exceed $473,419 to $1,030,090 for the term July 1, 2009 through June 30, 2011. This contract amendment also provides additional administrative changes to the Work Statement and Compensation sections. All other terms and conditions of the original contract shall remain in full force and effect. This contract is a subcontract to a grant from First Things First for the South Phoenix Nurse Family Partnership awarded to Maricopa County through its Department of Public Health in the amount of $1,165,358, and therefore does not affect the county general fund. Payment to contractor is on a cost reimbursement basis. 74. AMENDMENT TO CONTRACT WITH SOUTHWEST HUMAN DEVELOPMENT FOR COMPREHENSIVE COMMUNITY HEALTH NURSING SERVICES Approve Amendment No. 2 between Southwest Human Development (SWHD) and Maricopa County through its Department of Public Health (MCDPH) to extend the expiration date through June 30, 2012; increase the total, not- to-exceed, by $556,661 for the budget period July 1, 2011 through June 30, 2012, with a new aggregate total of $1,586,751; and to add Section III - Work Statement. All other terms and conditions of the original contract shall remain in full force and effect. This amendment is a grant from First Things First and does not affect the county general fund. This contract is included in FY12 budget recommendation. (C-86-10-003-3-02)
