21. IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR 7TH AVENUE WIC CLINIC BUILDING REPLACEMENT Approve the Intergovernmental Agreement (IGA) Contract No. (HG050026) between the Arizona Department of Health Services (ADHS) and Maricopa County and its Department of Public Health, Women, Infants and Children (WIC) Nutritional Services to provide modular building replacement for the 7th Avenue WIC Clinic site. The term shall be effective upon final signature and shall remain effective until one year thereafter, for a total amount of not-to-exceed $870,800. The Department of Public Health’s FY 2009-10 indirect rate is 19.5%. Total indirect costs are estimated at $11,533 all of which is recoverable. The contract total includes capital expenses for $800,000 not subject to indirect costs. Funds for this IGA are provided by ADHS and do not affect the County’s general fund. (C-86-10-005-3-00)

AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR 7TH AVENUE WIC CLINIC BUILDING REPLACEMENT Approve the Amendment No. 1 for Intergovernmental Agreement (IGA) Contract No. (HG050026) between the Arizona Department of Health Services (ADHS) and Maricopa County and its Department of Public Health, Women, Infants and Children (WIC) Nutritional Services to provide modular building replacement for the 7th Avenue WIC Clinic site. This Amendment will extend the end date to September 30, 2010. All other provisions shall remain unchanged. (C-86-10-005-3-01)
