20. CONTRACT AWARD FROM ARIZONA DEPARTMENT OF HEALTH SERVICES FOR NEWBORN INTENSIVE CARE PROGRAM Approve Notice of Award for Contract No. (HP961245-005) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) for High Risk Perinatal Program/Newborn Intensive Care Program Community Nursing. The term of the contract shall commence on July 1, 2009 and continue for a period of three years. The contract award is in the amount not-to-exceed $465,300 for the budget term beginning July 1, 2009 through June 30, 2010. The Department of Public Health indirect rate for FY 2009-10 is 19.5%. Indirect Cost are estimated at $75,928, all of which is fully recoverable. Funding for this grant will be provided by ADHS and will not affect the County general fund. The actual grant award is lower than FY 2009-10 Adopted budget of $699,574. A budget reduction for this grant will be included in the first true up agenda for appropriation adjustment. AMENDMENT TO CONTRACT WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIGH RISK PERINATAL PROGRAM/ NEWBORN INTENSIVE CARE PROGRAM COMMUNITY NURSING Approve Amendment No. 1 to Contract (No. HP961245-005) between Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) for High Risk Perinatal Program/Newborn Intensive Care Program Community Nursing. Amendment No.1 makes administrative changes to the Special Terms and Conditions and to the Scope of Work. This amendment also replaces the Price Sheet of original contract with revised Price Sheet with no change in contract amount. The term of the contract is July 1, 2009 through June 30, 2012. PROCUREMENT CHANGE ORDER TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIGH RISK PERINATAL PROGRAM/NEWBORN INTENSIVE CARE PROGRAM Approve Procurement Change Order to Purchase Order (E1H37101) for the Contract (HP961245-005-2) between the Arizona Department of Health Services (ADHS) and Maricopa County through its Department of Public Health (MCDPH) for the High Risk Perinatal Program/Newborn Intensive Care Program (HRPP/NICP) Community Nursing to increase funding by $30,000, for a new total not-to-exceed amount of $495,301, for the contract period July 1, 2010 to June 30, 2011. The contract term is from July 1, 2009 to June 30, 2012. The Department of Public Health’s indirect rate for FY2010-11 is 17%. The indirect expenses for the additional funding are fully recoverable for $4,359. Funding for this grant will be provided by ADHS and will not affect the County general fund. AMENDMENT TO IGA WITH ARIZONA DEPARTMENT OF HEALTH SERVICES FOR HIGH RISK PERINATAL PROGRAM/NEWBORN INTENSIVE CARE PROGRAM Approve Purchase Order (PO) No: (ADHS11-006208:6) for the Intergovernmental Agreement (IGA) Contract No. (ADHS11-006208) between the Arizona Department of Health Services (ADHS) and Maricopa County by and through its Department of Public Health (MCDPH), High Risk Perinatal Program (HRPP), Newborn Intensive Care Program, , to provide funding for the HRPP/Community Nursing. Reference PO ADHS11-0062808:6, Item 1, the PO effective budget period is from July 1, 2013 through June 30, 2014, for the annual contract amount $468,001. The Contract term is from July 1, 2009 to June 30, 2014. Terms and conditions set forth in the bid, quotation, or purchase order are incorporated herein by reference and become a part of this order. The Department of Public Health indirect rate for FY2013-14 is 15%. The indirect costs are estimated at $61,044, which are fully recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. §42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation.
