36. AGREEMENTS WITH TELEVOX SOFTWARE INC. FOR VACCINE REMINDER PROGRAM Approve a non-financial Service Agreement and Business Associate Agreement between TeleVox Software, Inc. and Maricopa County through its Department of Public Health's (MCDPH) Immunization Program for implementing a Vaccine Reminder Program. These agreements will allow TeleVox Software, Inc. to deliver the Wyeth-Sponsored Vaccine Reminder Program, a vaccine reminder callback and postcard program to clients of MCDPH’s Immunization Services. These agreements are non-financial and effective upon date of execution by the Board of Supervisors, and will remain in effect for a period of one year following execution. On August 14, 2009, Maricopa County issued a Notice of Intent (NOI) (Serial # PH NOI 10-009) for MCDPH’s intent to enter into an Agreement with TeleVox Software, Inc. As of the NOI deadline of September 1, 2009, 2pm MST, MCDPH’s Office of Procurement confirmed no other interest in response to this NOI. (C-86-10-027-3-00)
