27. CONTRACT WITH APOTHECARY SHOP FOR TAMIFLU AND RELENZA PRESCRIPTION FULFILLMENT SERVICES Approve contract (RFP PHA 10-007F) with Apothecary Shops for the purchase of Tamiflu and Relenza Prescription Fulfillment Services. The total not-to-exceed amount of all Contracts pursuant for PH RFP 10-007 will not exceed the amount of $223,688. This contract is funded by a grant from Arizona Department of Health Services (ADHS) (HG754199) This contractor was a successful respondent to a Request for Proposal (MC1-328) issued by the Maricopa County Department of Public Health pursuant to PH RFP 10-007. This contract is funded by a grant from (ADHS) (HG754199) and does not affect the county general fund. Reimbursement to subcontractors is per vaccination at a rate of $5 per prescription.
