40. CONTRACT WITH MARICOPA INTEGRATED HEALTH SYSTEMS FOR LABORATORY TESTING SERVICES Approve Request for Proposal (RFP) Contract (PH RFP 10-001A) between Maricopa County Special Healthcare District dba Maricopa Integrated Health Systems (MIHS) and Maricopa County through its Department of Public Health, Clinical Services to provide Laboratory Testing Services. The RFP contract period is from October 12, 2009 through October 11, 2012. MIHS is a successful respondent to a RFP (PH RFP 10-001) issued by Maricopa County Department of Public Health (MCDPH) pursuant to MC1-328. 75. AMENDMENT TO CONTRACT WITH MARICOPA INTEGRATED HEALTH SYSTEM FOR LABORATORY TESTING SERVICES Approve Amendment No. 1 to Request for Proposal (RFP) Contract (PH RFP 10-001A) between Maricopa County Special Healthcare District dba Maricopa Integrated Health Systems (MIHS) and Maricopa County through its Department of Public Health, Clinical Services to provide Laboratory Testing Services. This Amendment will extend the RFP contract period for an additional two (2) years from October 12, 2012 through October 11, 2014. All other terms and conditions of the original contract shall remain in full force and effect. MIHS is a successful respondent to a Request for Proposal (PH RFP 10-001) issued by Maricopa County Department of Public Health (MCDPH) procured under Section MC1-330 of the Maricopa County Procurement Code Competitive Sealed Proposals. The contract will be funded through various sources, grant fund (532), fee fund (265) and general funds (100) from the Clinical Services Division throughout the contract period. (C-86-10-101-3-01)

AMENDMENT TO CONTRACT WITH MARICOPA COUNTY SPECIAL HEALTHCARE DISTRICT D/B/A MARICOPA INTEGRATED HEALTH SYSTEMS FOR LABORATORY TESTING SERVICES Approve Amendment No. 2 to Request for Proposal (RFP) Contract (PH RFP 10-001A) between Maricopa County Special Healthcare District d/b/a Maricopa Integrated Health Systems (MIHS) and Maricopa County through its Department of Public Health, Clinical Services to provide Laboratory Testing Services. This Amendment under 1.0 Contract Term, 1.2., the original contract, at the County’s sole discretion, extend the contract on a month-to-month basis for a maximum of six (6) month after expiration, beginning on October 12, 2014 and ending April 11, 2015. All other terms and conditions of the original contract shall remain in full force and effect. MIHS is a successful respondent to a Request for Proposal (PH RFP 10-001) on October 12, 2009 issued by Maricopa County Department of Public Health (MCDPH) procured under Section MC1-330 of the Maricopa County Procurement Code Competitive Sealed Proposals.
