45. CONTRACT WITH FRYS FOOD DRUG STORES FOR COMMUNITY IMMUNIZATION PROVIDER SERVICES Approve Contract to a Public Health Request for Proposal 10-005 (PH RFP 10-005H) between Fry’s Food Drug Stores (FFDS) and Maricopa County through its Department of Public Health (MCDPH) for the purpose of providing Community Immunization Provider (CIP) Services. The total for all contracts awarded under PH RFP 10-005 cannot exceed the combined aggregate total of $1,505,463. The term for this contract is from November 12, 2009 ending November 11, 2010. FFDS was a successful respondent to Request for Proposal (PH RFP 10-005) issued by the MCDPH pursuant to MC1-328. This Contract is funded by a grant from Arizona Department of Health Services (AHDS) (HG754199) and does not affect the county general fund. This contract is a subcontract to IGA (HG754199) between Maricopa County through its Department of Public Health and the Arizona Department of Health Services (ADHS) and does not affect the county general fund. Reimbursement to subcontractors is per vaccination at a rate of $8 per vaccination administered. The total not-to-exceed amount of all contracts pursuant to PH RFP 10-005 will not exceed the amount of $1,505,463. (C-86-10-105-3-00)

AMENDMENT TO CONTRACT WITH FRYS FOOD DRUG STORES PROVIDING H1N1 IMMUNIZATIONS TO THE PUBLIC Approve Amendment No. 1 to the Contract between Frys Food Drug Stores Healthcare and Maricopa County through its Department of Public Health, to extend the expiration date from November 11, 2010 to November 11, 2015. All other terms and conditions of this Contract remain in full effect. (C-86-10-105-3-01)
