79. ADMINISTRATIVE CORRECTION TO LEASE AGREEMENT G-30122 WITH MARICOPA INTEGRATED HEALTH SYSTEM FOR AVONDALE WIC SITE Approve an Administrative Correction to action approved on January 14, 2010 (C-86-10-106-1-00) to execute Lease No. G-30122 (C-90-10-007-1) with Maricopa County Special Health Care District d.b.a. Maricopa Integrated Health System (MIHS), Lessor, for 2,619 square feet (sf) of office space location at the Avondale Family Health Center (FHC) located at 950 East Van Buren Street, Avondale, AZ 85323. The rental rate is $17.00 sf or an annual rate of $44,523 plus rental tax. The initial term of this lease shall be three (3) years and shall commence upon execution by Lessee and Lessor with no provision for “hold over”. At the end of the initial term, this lease shall automatically renew for seven (7) additional one (1) year periods for a total of ten (10) years, unless terminated in accordance with paragraph 8 of this lease. The Administrative Correction will add the following language, after the initial term of the contract, each subsequent renewal term shall be subject to a rate increase of 3%. (C-86-10-106-1-01) 80. GRANT FOR QUALITY IMPROVEMENT AWARD WITH NATIONAL NETWORK OF PUBLIC HEALTH INSTITUTES Approve Grant for a Quality Improvement Award (C490), Nation Network of Public Health Institutes for Maricopa County by and through its Department of Public Health, Performance Improvement to develop a program to meet the accreditation standards and measures set by the Public Health Accreditation Board (PHAB). The award is for a not-to-exceed amount of $5,000 for an eight (8) month period from December 1, 2012 to August 31, 2013. This grant deviates from County policy A2505 and does not allow for full indirect cost reimbursement, The Department of Public Health’s indirect rate for FY 2012-2013 is 15.8%. Total indirect expenses based on full award are estimated to be $790, all of which is not recoverable. Grant revenues are not local revenues for the purpose of the constitutional expenditure limitation, and therefore expenditure of the revenues is not prohibited by the budget law. This Amendment does not alter the budget constraining expenditures of local revenues duly adopted by the Board pursuant to A.R.S. 42-17105. The overall grant budget will be adjusted as necessary to accommodate this grant through a future reconciliation. (C-86-13-008-G-01)

48. TERMINATION OF IGA LEASE BY AND BETWEEN MARICOPA SPECIAL HEALTH CARE DISTRICT AND MARICOPA COUNTY Approve the termination of Intergovernmental Lease Agreement (G-30122) by and between Maricopa Special Health Care District (Lessor) and Maricopa County (Lessee) for use of property at 950 East Van Buren Street, Avondale, AZ, due to non-appropriation of funds and authorize Real Estate Division staff to give formal notice to Lessor of the Lease termination. Section 11 of the Lease allows the County the option to terminate the Lease at the end of any fiscal year for non-appropriation of funds without penalty. Federal funding for this WIC clinic location will end on September 30, 2017. The termination will be effective on, or before September 30, 2017. (C-86-10-106-1-02)
