24. IGA WITH ARIZONA BOARD OF REGENTS FOR DECISION SUPPORT LOGISTICS AND DISTRIBUTION OF H1N1 VACCINES Approve the Intergovernmental Agreement (IGA) (ASU No. 10-046461) with Arizona Board of Regents for an on behalf of Arizona State University (ASU) and its WP Carey School of Business and Maricopa County through its Department of Public Health (MCDPH). This IGA will begin on January 1, 2010 and will remain in effect unless either party provides 30 days written notice. Also, approve this IGA’s Task Order Modification 1 with MCDPH’s Office of Preparedness and Response (OPR) for Decision Support for Logistics and Distribution of H1N1 Vaccines. The term of Task Order Modification 1 is October 1, 2009 through August 31, 2010, unless modified in writing by both parties for the amount of $149,999. Funds for IGA’s Task Order Modification 1 are supported by a grant from ADHS (HG754199) and do not increase the county general fund. (C-86-10-110-3-00)

This Amendment extends the expiration date to December 31, 2013; Revises Section 1, Number 1, Scope of Work adds two an additional Scope of Works and Section 1, Number 4, Compensation is changed to $437,161, (Increased by $90,500), Scope of Work 1: $50,000 for budget period September 1, 2012 to June 30, 2013 and Scope of Work 2: $40,500 for budget period September 1, 2012 to December 31, 2013. This Amendment extends the expiration date to December 31, 2014 and Section 1, 4. Compensation is increased by $72,413 for a new contract total of $509,574. AMENDMENT TO IGA WITH ARIZONA BOARD OF REGENTS FOR AND ON BEHALF OF ARIZONA STATE UNIVERSITY FOR H1N1 LOGISTICAL SUPPORT Approve Amendment No. 8 to the Intergovernmental Agreement (IGA) between Arizona Board of Regents for an on behalf of Arizona State University (ASU) and Maricopa County by and through its Department of Public Health (MCDPH). This Amendment per Section I, Article III, Special Provisions, 1. Compensation for the budget period 01/01/2015 through 12/31/2015 will be a fixed rate in the amount of $5,000. The contract period is January 1, 2015 through December 31, 2015. All other terms and conditions of the original agreement shall remain in full force and effect. (C-86-10-110-3-09)
