19. IGA WITH SCOTTSDALE UNIFIED SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve an Intergovernmental Agreement (IGA) between Scottsdale Unified School District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health. Funding is for a not-to-exceed amount of $9,200 for budget period beginning July 1, 2009 through May 1, 2010. Funding for this agreement is provided by the Tobacco Education/Prevention Grant from ADHS and will not increase the County general fund budget. (C-86-10-112-3-00)

AMENDMENT TO IGA WITH SCOTTSDALE UNIFIED SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 1 to Intergovernmental Agreement (IGA) between Scottsdale Unified School District and Maricopa County through its Department of Public Health to increase the contract by $8,050 to provide school-based tobacco use prevention and education services for contract period ending May 1, 2011. This amendment brings the IGA to a new not-to-exceed aggregate amount of $17,250 for the contract term July 1, 2009 through May 1, 2011. This amendment brings the IGA to a new not-to-exceed aggregate amount of $40,250 for the contract term July 1, 2009 through May 1, 2012. AMENDMENT TO IGA WITH SCOTTDALE UNIFIED HIGH SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 3 to Intergovernmental Agreement (IGA) between Scottsdale Unified School High School District and Maricopa County through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health, which increases funding by $4,000 for budget period July 1, 2012 through May 1, 2013, for a new aggregate not-to-exceed contract total of $44,250. The expiration date is changed to May 1, 2013; this amendment also replaces Section I, 22. Equal Employment Opportunities, Section III, 2 Service Goals, Section III, 3 Services Objectives/Activities, and revise Section III, 4 Other Requirements, B, and list of Participating Schools in Attachment A. This Amendment is effective upon execution by both parties. All other terms and conditions of the original Agreement that have not been amended by this or previous amendments shall remain in full force and effect. Funds for this IGA are provided by the Tobacco Education Prevention Grant from Arizona Department of Health Services (ADHS) and will not affect the County’s general fund. (C-86-10-112-3-03)

AMENDMENT TO IGA WITH SCOTTSDALE UNIFIED HIGH SCHOOL DISTRICT FOR SCHOOL-BASED TOBACCO USE PREVENTION AND EDUCATION SERVICES Approve Amendment No. 4 to Intergovernmental Agreement (IGA) between Scottsdale Unified School District (SUSD) and Maricopa County through its Department of Public Health (MCDPH) to provide school-based tobacco use prevention and education services. This Amendment increases funding by $1,000 for budget period July 1, 2012 through June 30, 2013, for a new aggregate not-to-exceed contract total of $45,250. The expiration date is changed to June 30, 2013; this Amendment also replaces Section 1, 22. Equal Employment Opportunities, Section III, 2. Service Goals, Section III, 3. Services Objectives/Activities and revise Section III, 4. Other Requirements B., and List of Participating Schools in Attachment A. This Amendment is effective upon execution by both parties. All other terms and conditions of the original Agreement that have not been amended by this or previous amendments shall remain in full force and effect.
