IGA WITH PARADISE VALLEY UNIFIED SCHOOL DISTRICT FOR TOBACCO USE PREVENTION AND EDUCATION Approve an Intergovernmental Agreement (IGA) between Paradise Valley Unified School District and Maricopa County by and through its Department of Public Health to provide school-based tobacco use prevention and education services for the Maricopa County Department of Public Health. Funding is for a not-to-exceed amount of $13,000 for budget period beginning July 1, 2009 through May 1, 2010. Funding for this agreement is provided by the Tobacco Education/Prevention Grant from ADHS and will not increase the County general fund budget. (C-86-10-134-3-00)

This amendment brings the IGA to a new not-to-exceed aggregate amount of $26,000 for the contract term July 1, 2009 through May 1, 2011. 42. AMENDMENT TO IGA WITH PARADISE VALLEY UNIFIED SCHOOL DISTRICT FOR SCHOOL HEALTH ASSESSMENT PROGRAM Approve Amendment No. 3 to Intergovernmental Agreement (IGA) between Paradise Valley Unified School District and Maricopa County through its Department of Public Health (MCDPH), Maricopa County Tobacco and Chronic Disease to provide funding for a School Health Assessment Program. This amendment extends the expiration date to May 1, 2013 and to increase the contract by $8,000, for budget period July 1, 2012 through May 1, 2013, for a new not-to-exceed aggregate amount of $46,000 for the contract term July 1, 2009 through May 1, 2013. Additional administrative changes are included in this amendment. Funding for this agreement is provided by the Tobacco and Chronic Disease Grant from Arizona Department of Health Services and will not affect the County general fund budget. (C-86-10-134-3-03)
