FACILITY USE/REVOCABLE LICENSE AGREEMENT WITH CATHOLIC HEALTHCARE WEST DBA CHANDLER REGIONAL MEDICAL CENTER FOR ORAL HEALTH SERVICES Approve full service Facilities Use/Revocable Agreement No. P50136 with Catholic Healthcare West, a California non-profit public benefit Corporation d/b/a Chandler Regional Medical Center, Licensee, for the user of clinic space to perform Oral Health Services to uninsured WIC eligible children ages 0 to five, their families and pregnant women, at the WIC clinic located at 3002 North Arizona Avenue, Chandler, AZ 85225. The term of the Agreement is from June 1, 2010 through May 31, 2011. License agrees to provide the services at no cost to the County and the recipients. 39. AMENDMENT TO FACILITY USE AGREEMENT WITH CATHOLIC HEALTHCARE WEST D/B/A CHANDLER REGIONAL MEDICAL CENTER Approve Amendment No. 1 to full service Facilities Use/Revocable Agreement No. P50136 between Catholic Healthcare West, a California non-profit public benefit Corporation d/b/a Chandler Regional Medical Center, Licensee, and Maricopa County by and through its Department of Public Health, WIC Agency, for the user of clinic space to perform Oral Health Services to uninsured WIC eligible children ages 0 to five, their families and pregnant women, at the WIC clinic located at 3002 North Arizona Avenue, Chandler, AZ 85225 . The term of the Agreement is from June 1, 2010 through May 31, 2011. This Amendment adds the following paragraph to Section 10. Insurance: 10.8 The User shall have in effect at all times during the term of this Agreement insurance or comparable self-insurance that is adequate to protect the County, its officers, employees, property, and equipment against the losses set forth below. The User shall provide the County with a certificate of insurance or a certified copy of the insurance policy naming the County as an additional insured, or, if the User is self-insured, shall provide the County with a letter indicating that it is self-insured. All other terms and conditions of the original Agreement shall remain in full force and effect. (C-86-10-141-M-01)

AMENDMENT TO FACILITY USE REVOCABLE LICENSE AGREEMENT WITH DIGNITY HEALTH FOR ORAL HEALTH, EDUCATION, HEARING AND VISION SCREENINGS AT CHANDLER WIC Approve Amendment No. 4 to full service Facilities Use/Revocable License Agreement (FURL) (P50136) between the Dignity Health, a California non-profit public benefit corporation d/b/a Chandler Region Medical Center (CRMC), Licensee, and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Agency, for the use of clinic space to perform Oral Health, Hearing and Vision screening and; Oral Health Education to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 3002 North Arizona Avenue, Chandler, AZ 85225.. The Agreement term is extended one (1) year, commencing June 1, 2013 to May 31, 2014. This amendment also removes and replaces Exhibit A. All other terms and conditions of the original agreement and its amendments shall remain in full force and effect. The Facility Review Committee has reviewed this Facility Use/Revocable Agreement at the March 2013 meeting. This Use Permit Agreement also allows for CRMC the use of clinic space at the Mesa WIC facility as scheduled on Exhibit “A”. Licensee agrees to provide the services at no cost to the County and the recipients. County agrees to provide access without monetary remuneration. (C-86-10-141-M-03)

AMENDMENT TO LICENSE AGREEMENT WITH DIGNITY HEALTH, D/B/A CHANDLER REGIONAL MEDICAL CENTER FOR CHANDLER WIC CLINIC SPACE Approve and authorize the Chairman to execute, Amendment No. 5 Facilities Use Revocable License Agreement (FURL) P-50136 between Dignity Health, a California non-profit public benefit corporation, d/b/a Chandler Regional Medical Center (CRMC), Licensee, and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Agency, for the use of clinic space to perform oral health, hearing and vision screening and oral health education to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 3002 North Arizona Avenue, Chandler, Arizona. The term will be extended one (1) year to commence June 1, 2014 and expire May 31, 2015. This amendment also updates Exhibit “A” with new dates and times. All other terms and conditions of the original agreement and its amendments remain the same and in full force and effect. This Facility Use Agreement also allows for CRMC the use of clinic space at the Chandler WIC facility as scheduled on Exhibit “A”. Licensee will provide services at no cost to Maricopa County and the recipients. County agrees to provide access without remuneration. This general type of Real Estate action was endorsed by the Facilities Review Committee during August of 2013. (C-86-10-141-M-04)

AMENDMENT TO LICENSE AGREEMENT WITH DIGNITY HEALTH, DBA CHANDLER REGIONAL MEDICAL CENTER FOR CHANDLER WIC CLINIC SPACE Approve and authorize the Chairman to execute, Amendment No. 6 Facilities Use Revocable License Agreement (FURL) P-50136 between Dignity Health, a California non-profit public benefit corporation, d/b/a Chandler Regional Medical Center (CRMC), Licensee, and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Agency, for the use of clinic space to perform oral health, hearing and vision screening and oral health education to underserved WIC eligible children ages zero (0) to five (5), their families and pregnant women at the WIC clinic located at 3002 North Arizona Avenue, Chandler, Arizona. The term will be extended seven (7) months to commence June 1, 2015 and expire December 31, 2015. This amendment also updates Exhibit “A” with new dates and times. All other terms and conditions of the original agreement and its amendments remain the same and in full force and effect. This Facility Use Agreement also allows for CRMC the use of clinic space at the Chandler WIC facility as scheduled on Exhibit “A”. Licensee will provide services at no cost to Maricopa County and the recipients. County agrees to provide access without remuneration. The short renewal term allows a transition to a new program approach to deliver oral health services to the WIC clients. (C-86-10-141-M-05)
