30. CONTRACTS FOR BREAST AND CERVICAL CANCER SCREENING SERVICES Approve four (4) FY 2011-2013 Contracts for the purpose of purchasing Breast and Cervical Cancer Screening Services and/or diagnostic and treatment services to women 40 years of age and older who are either uninsured or underinsured and whose household income falls at or below 250% of the Federal Poverty Level Guidelines. These Contracts are funded by a grant from Arizona Department of Health Services (ADHS), (HG761266). Each of these contractors was a successful respondent to a Request for Proposal issued by the Maricopa County Department of Public Health (PH RFP 10-023). Contractors and Serial Numbers are as follows: Mountain Park Health Center (Serial #: 860-10-23A) (C-86-10-147-3-00) Adelante Healthcare (Serial #: 860-10-23B) (C-86-10-148-3-00) Wesley Community Center (Serial #: 860-10-23C) (C-86-10-149-3-00) Contemporary Care for Women (Serial #: 860-10-23D) (C-86-10-150-3-00) Funding for these contracts is provided through an IGA (HG761266) between Maricopa County through its Department of Public Health, and ADHS, and does not increase the county general fund. Reimbursement to subcontractors is on a per enrollee basis at a rate established by ADHS. Additional funding is provided by the Phoenix Affiliate of the Susan G. Komen Breast Cancer Foundation and does not increase the county general fund. This funding augments Well Woman Healthcheck services and provides additional funds for diagnostic and treatment services. (C-86-10-151-3-ZZ)

These Contracts are funded by a grant from Arizona Department of Health Services, (HG761266). The Contracts are for a term of three years, beginning on August 1, 2011 and ending July 31, 2014. Each of these contractors was a successful respondent to a Request for Proposal issued by the Maricopa County Department of Public Health (PH RFP 11-031). Contractors and Serial Numbers are as follows: Providence General Medicine (Serial #: 860-11-31A) (C-86-10-147-3-00) Native American Community Health Center (Serial #: 860-11-31B) (C-86-10-148-3-00) Funding for these contracts is provided through an IGA (HG761266) between Maricopa County through its Department of Public Health, and ADHS, and does not increase the county general fund. 55. AMENDMENT TO CONTRACT WITH MOUNTAIN PARK HEALTH CENTER FOR WELL WOMAN HEALTHCHECK SERVICES Approve Amendment No. 1 to the Contract between Mountain Park Health Center (MPHC) and Maricopa County by and through its Department of Public Health (MCDPH) to provide Well Woman Healthcheck (WWHC) services to uninsured and underinsured women and whose income falls at or below 250% of the Federal Poverty Level Guidelines. This contract amendment, under 1.0 Contract Term, 1.1, is extending the end date to December 31, 2013. This contract was procured under Section MC1-328 of the Maricopa County Procurement Code, Competitive Sealed Proposals and was a successful respondent to a Request for Proposals issued by the Department of Public Health (PH RFP 10-023) on July 1, 2010. Also, this contract is funded through an Arizona Department of Health Services (ADHS) grant, WWHC (ADHS13-029502) and will not affect the county general fund. Reimbursement to the contract is on a per enrollee basis at a rate established by ADHS. (C-86-10-147-3-01)
