29. PARTNERSHIP AGREEMENT WITH UMOM NEW DAY CENTERS INC FOR WIC ENROLLMENT SERVICES Approve a non-monetary Partnership Agreement (PA) between, a non-profit organization, UMOM New Day Centers, Inc. (UMOM) and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children Services (WIC) to provide WIC enrollment services to the WIC eligible UMOM clients. WIC enrollment will be conducted on UMOM premises at 3333 East Van Buren, Phoenix, AZ, 85008. The PA will be in effect from June 1, 2010 to September 30, 2013. (C-86-10-153-M-00)

37. AMENDMENT TO PARTNERSHIP AGREEMENT WITH UMOM NEW DAY CENTERS, INC FOR WIC ENROLLMENT SERVICES Approve Amendment No. 1 to non-monetary Partnership Agreement (PA) between UMOM New Day Centers, Inc. (UMOM) and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children Services (WIC) to provide WIC enrollment services to the WIC eligible UMOM clients. This Amendment will be extend the end date to September 30, 2016. Also, this Amendment will update the administrative contact person and Insurance terms. All other terms and conditions of the original agreement and its amendments shall remain in full force and effect. The Women, Infants, and Children Supplemental Food Nutrition Education Program (WIC) is funded by the U.S. Dept. of Agricultural and administered through ADHS. The program provides vouchers for approved foods (milk, cheese, cereals, peanut butter, eggs, infant formula, juice and cereal etc.) for low-income pregnant, post partum, breast-feeding women, infants and children up to five years of age. Clients also receive nutrition and breastfeeding education and referrals to applicable social services. The program serves over 90,000 clients per month at sites across the County and distributes approximately $60 million in vouchers annually for redemption in local grocery stores. (C-86-10-153-M-01)

48. AMENDMENT TO PARTNERSHIP AGREEMENT WITH UMOM NEW DAY CENTERS, INC FOR WIC ENROLLMENT SERVICES Approve Amendment No. 2 to terminate this non-monetary Partnership Agreement (PA) between UMOM New Day Centers, Inc. (UMOM) and Maricopa County by and through its Department of Public Health (MCDPH), Women, Infant and Children (WIC) Services to provide WIC enrollment services to the WIC eligible UMOM clients. Pursuant to Section F. IT IS MUTUALLY UNDERSTOOD AN AGREED BY AND BETWEEN THE PARTIES THAT, under paragraph 3. TERMINATION. Either Party, in writing, may terminate the instrument in whole or in part at any time before the expiration. MCDPH WIC Services will be exercising this option to terminate the instrument in whole. The term of the PA is September 30, 2016. Also, The termination of the PA will be effective upon final execution. The WIC eligible UMOM clients have two other local WIC locations within the geographic area to obtain WIC services. (C-86-10-153-M-02)
