16. CONTRACT WITH THE ARIZONA PARTNERSHIP FOR IMMUNIZATION/ARIZONA ASSOCIATION OF COMMUNITY HEALTH CENTERS Approve Contract Pursuant to RFP for the purpose of Community Immunization Education and Outreach Services with The Arizona Partnership for Immunization/Arizona Association of Community Health Centers (TAPI). The total for this contract awarded under PH RFP 10-029 is in amount not-to-exceed $245,500 for the term June 9, 2010 through June 8, 2011, with an option to renew the contract for a total of five years. This contractor was a successful respondent issued by the Maricopa County Department of Public Health pursuant to PH RFP 10-007. This Contract is funded by a grant from Arizona Department of Health Services (AHDS) (HG754199) and does not affect the county general fund. This agreement is a subcontract to IGA (HG754199) between Maricopa County through its Department of Public Health, and the Arizona Department of Health Services (ADHS) and does not increase the county general fund. AMENDMENT TO CONTRACT WITH THE ARIZONA PARTNERSHIP FOR IMMUNIZATION Approve Amendment No. 1 to the Contract with The Arizona Partnership for Immunization (TAPI) for Community Immunization Education and Outreach Services to increase the funding by $375,000, to a not-to-exceed, amount of $620,500. This amendment is for the budget period June 9, 2011 to June 8, 2012. This amendment revises the scope of work and extends the expiration date from June 8, 2011 to June 8, 2015. AMENDMENT TO CONTRACT WITH THE ARIZONA PARTNERSHIP FOR IMMUNIZATION/ARIZONA ASSOCIATION OF COMMUNITY HEALTH CENTERS Approve Amendment No. 2 to the Contract with The Arizona Partnership for Immunization/Arizona Association of Community Health Centers (TAPI) for Community Immunization Education and Outreach Services to increase the funding by $30,000, to a not-to-exceed, amount of $650,500, for the budget period June 9, 2011 to June 8, 2012. This amendment also revises the scope of work. All other terms and condition of the original contract remain in full force and effect.
