AMENDMENT TO CONTRACT WITH THE CENTER FOR AFRICAN AMERICAN HEALTH ARIZONA TO PROVIDE HARD TO REACH POPULATION SPECIALIST SERVICES Approve Amendment No. 1 to the Contract between the Center for African American Health Arizona and Maricopa County through its Department of Public Health, to extend the expiration date from June 22, 2011 to June 22, 2012. AMENDMENT TO CONTRACT WITH THE CENTER FOR AFRICAN AMERICAN HEALTH ARIZONA FOR HARD TO REACH POPULATION SPECIALIST SERVICES Approve Amendment No. 2 to the Contract between the Center for African American Health, Arizona and Maricopa County through its Department of Public Health, to add an additional Scope of Work and increase the funding by $9,000, for the budget year June 23, 2011 through June 22, 2012, for a new total, not-to-exceed amount of $190,000 for the Contract term June 23, 2010 through June 22, 2012. All other terms and conditions of this Contract remain in full effect. (C-86-10-167-3-02) 52. AMENDMENT TO LETTER OF AGREEMENT WITH MARICOPA INTEGRATED HEALTH SYSTEM FOR STD TESTING Approve Amendment No. 1 to an Intergovernmental Letter of Agreement (LOA) between Maricopa County Integrated Health Systems (MIHS) and Maricopa County through its Department of Public Health to provide Chlamydia and Gonorrhea testing of MIHS patients. The Term on the Contract is to be amended January 7, 2011 through January 6, 2013 and the effective date is January 7, 2012.
