25. CONTRACT RENEWAL WITH ASIAN PACIFIC COMMUNITY IN ACTION Approve the extension of the Contract Pursuant to PH-RFP 09-013 with Asian Pacific Community in Action for the purpose of continuing the development and implementation of emergency preparedness plans for hard to reach populations (HRP). This contract is in an amount not-to-exceed $157,900 for a one year term staring July 1, 2010 through June 30, 2011. All other terms and conditions of the original contract remain unchanged. The original contract was approved on December 2, 2008 through Materials Management for the term November 5, 2008-November 4, 2009, for a not-to-exceed amount of $50,000. The contract was extended on August 17, 2009, by the Department of Public Health for an additional one year term through June 30, 2010. (C-86-10-172-3-00)

AMENDMENT TO CONTRACT WITH THE ASIAN PACIFIC COMMUNITY IN ACTION FOR HARD TO REACH POPULATION SPECIALIST SERVICES Approve Amendment no. 2 to the Contract between the Asian Pacific Community in Action and Maricopa County through its Department of Public Health, to add an additional Scope of Work and increase the funding by $10,010, for the budget year July 1, 2011 through June 30, 2012, for a new total, not-to-exceed amount of $167,910 for the Contract term July 1, 2010 through June 30, 2012.
